目的 观察白内障超生乳化术中房角钝性分离治疗合并闭角型青光眼的白内障患者的临床效果.方法 对2012年7月至2013年10月在荷泽华厦眼科医院就诊的白内障合并闭角型青光眼患者,且UBM显示房角关闭粘连小于两个象限者28例30只眼,均在表面麻醉下行透明角膜切口的白内障超生乳化联合1％玻璃酸钠(爱维)房角钝性分离术.结果 术后仅3只眼在一周内眼压轻度升高(22～28 mmHg),经降眼压药物治疗后眼压降至正常,随访6个月观察眼压变化情况,其中有2只眼眼压一直在25～30 mmHg,行小梁切除手术治疗.术后视力较术前明显提高,中央前房深度、房角与术前相比较均有不同程度改善.结论 对于部分白内障合并闭角型青光眼的患者,超生乳化联合房角钝性分离是一种便捷、安全、有效的方法,具有降压和增视的双重效果.

목적 관찰백내장초생유화술중방각둔성분리치료합병폐각형청광안적백내장환자적림상효과.방법 대2012년7월지2013년10월재하택화하안과의원취진적백내장합병폐각형청광안환자,차UBM현시방각관폐점련소우량개상한자28례30지안,균재표면마취하행투명각막절구적백내장초생유화연합1％파리산납(애유)방각둔성분리술.결과 술후부3지안재일주내안압경도승고(22～28 mmHg),경강안압약물치료후안압강지정상,수방6개월관찰안압변화정황,기중유2지안안압일직재25～30 mmHg,행소량절제수술치료.술후시력교술전명현제고,중앙전방심도、방각여술전상비교균유불동정도개선.결론 대우부분백내장합병폐각형청광안적환자,초생유화연합방각둔성분리시일충편첩、안전、유효적방법,구유강압화증시적쌍중효과.
Objective To observe the clinical effect of cataract phacoemulsification surgery in the treatment of angle blunt separation combined with angle closure glaucoma with cataract.Methods The patients with cataract and angle closure glaucoma, angle closure adhesion and UBM showed less than two quadrants in 30 cases (28 eyes) of cataract phacoemulsification combined in 1％ sodium hyaluronate clear corneal incision under surface anesthesia (IVI) angle blunt dissection.Results Only 3 eyes within a week of intraocular pressure increased slightly (22-28mmHg), after treatment with drugs to lower intraocular tension, the intraocular pressure returned to normal, and followed up for 6 months to observe the intraocular pressure changes, among them 2 eyes intraocular pressure was 25-30mmHg, underwent trabeculectomy operation treatment.The postoperative visual acuity was improved obviously, central anterior chamber depth, angle and preoperative compared had different degrees of improvement.Conclusions For the cataract patients with angle closure glaucoma patients, phacoemulsification combined angle blunt separation method is a convenient, safe, effective, and have the effects of antihypertensive and amblyopia.
