目的 观察SD大鼠腰动脉结扎脊髓缺血损伤后Fas、含半胱氨酰天冬氨酸特异性蛋白酶-3(Caspase-3)、肿瘤坏死因子-α(TNF-α)、核因子-κB(NF-κB)、神经生长因子(NGF)、生长相关蛋白-43(GAP-43)及脑源性神经生长因子(BDNF)表达的变化,探讨SD大鼠脊髓缺血性损伤后自我修复的过程.方法 56只SD大鼠随机分为缺血组和假手术组.假手术组14只,缺血组42只,缺血组分别再随机分为缺血1、2、4、6、8、12和24h组,以假手术组相同阶段的脊髓为对照,采用神经功能评定法(BMS评分)评估各组行为学的变化,并采用免疫组织化学方法检测脊髓中Fas、Caspase-3、TNF-α、NF-κB、NGF、GAP-43及BDNF的表达变化.结果 缺血组SD实验大鼠BMS评分在缺血后立即出现显著降低[(0.58 ±0.66)分],至缺血后24h逐渐恢复至正常水平[(8.33±0.61)分],与假手术组比较差异无统计学意义(P>0.05),免疫组织化学染色结果显示Fas、Caspase-3、TNF-α、NF-κB表达的阳性细胞数在缺血后立即显著增多,其值分别为(45.50±4.18、47.50±3.27、53.50±7.71、57.83±4.79),与假手术组比较差异均有统计学意义(P<0.05),除Fas外(7.50±1.87),其余各个指标至缺血后24h逐渐恢复至正常水平,与假手术组比较差异均无统计学意义(P>0.05).GAP-43在缺血后阳性表达细胞数立即增多(6.33±1.21),缺血后各组与假手术组比较差异均有统计学意义(P<0.05),NGF在缺血后8 h(37.33±1.37)阳性表达细胞数才开始增多,随后逐渐升高,缺血后8、12、24 h与假手术组比较均有统计学意义(P<0.05),BDNF阳性表达细胞数在缺血后1、2h(22.83±1.94、23.83±1.94)显著低于假手术组,于缺血后4 h(31.33±2.16)开始上升,随后逐渐升高,缺血后4、6、8、12、24 h与假手术组比较差异均有统计学意义(P<0.05).结论 SD大鼠全部腰动脉结扎致脊髓缺血性损伤后24 h可恢复至正常水平,Fas、Caspase-3、TNF-α、NF-κB表达降低及NGF、GAP-43、BDNF表达增强可能与其自我恢复机制有关.

Objective To investigate the change of cysteinyl aspartate-specific protease-3 (Caspase-3), tumor necrosis factor-α (TNF-α), nuclear factor-κB (NF-κB), nerve growth factor (NGF), growth associated protein 43 (GAP-43), and brain derived neurotrophic factor (BDNF) expression after spinal cord ischemia injury in the SD rats by ligating lumbar arteries, and observe the process of self-repair after spinal cord ischemia injury in the SD rats.Methods Fifty-six SD rats were divide into ischemia group (n =42) and sham operation group (n =12) randomly.The SD rats in ischemia group were subdivided into 1,2, 4, 6, 8, 12 and 24 h subgroups.BMS score was adopted to assess the change of behavior.The expression of Fas, Caspase-3, TNF-α, NF-κB, NGF, GAP-43 and BDNF was detected by immunohistochemistry.Results In ischemia group, BMS score was obvious decrease immediately after ischemia injury (0.58 ±0.66) and return to normal level (8.33 ±0.61) 24 h after ischemia injury, there was no significant difference compared with control group (P > 0.05).Immunohistochemical study showed obvious increase were found in Fas, Caspase-3, TNF-α, NF-κB activity in ischemia group, the value were (45.50 ±4.18, 47.50 ±3.27, 53.50 ±7.71,57.83 ±4.79) respectively,which had statistically difference compared with control group (P < 0.05), and return to normal level 24 h after ischemia injury except Fas (7.50 ± 1.87).GAP-43 positive cell number increased immediately after ischemia injury (6.33 ± 1.21) and had statistically difference compared with control group in each group (P <0.05).The number of NGF positive cell were increase 8 h after ischemia injury (37.33 ± 1.37) and in 8, 12, 24 h after ischemia injury there had statistically difference compared with control group.BDNF activity cell number were significantly lower than control group 1 h and 2 h after ischemia injury (22.83 ± 1.94, 23.83 ± 1.94) and increased 4 h after ischemia injury (31.33 ±2.16), compared with control group there were significant difference (P < 0.05) at 4, 6, 8, 12, 24 h after ischemia injury.Conclusion Ischemia injury in spinal cord would repair 24 h after ischemia in SD rats with ligation of all lumbar arteries.The mechanism of self-repair may be related to the down-regulation of Fas, Caspase-3, TNF-α, NF-κB expression and the up-regulation of NGF, GAP-43, and BDNF expression.
