目的:探讨肥胖易患基因FTO基因位点rs8050136多态性与汉族人群脑梗死的相关性。方法选取2011年6月—2012年7月在中南大学湘雅三医院神经内科住院的脑梗死患者351例(脑梗死组),另选取同期在本院健康管理中心体检的汉族健康人群417例(对照组),收集受试者基本资料,并采集其血液,应用聚合酶链反应( PCR)及飞行时间质谱技术( MALDI-TOF-MS)对FTO基因位点rs8050136进行基因分型检测。结果对照组和脑梗死组性别、年龄、体质指数( BMI)、低密度脂蛋白胆固醇( LDL-C)、高密度脂蛋白胆固醇( HDL-C)、总胆固醇( TC)比较,差异均无统计学意义( P ﹥0.05);两组糖尿病病史、冠心病病史、高血压病史、体质量、收缩压(SBP)、舒张压(DBP)、三酰甘油(TG)、尿酸(UA)和空腹血糖(FBG)比较,差异均有统计学意义(P﹤0.05)。对照组和脑梗死组基因型分布和等位基因频率比较,差异均无统计学意义( P﹥0.05)。单因素线性回归分析结果显示,基因位点纯合子AA分别与BMI(b=0.004,b'=0.088,t=2.420,P=0.016)、体质量(b=0.001,b'=0.072, t=1.993,P=0.047)存在线性回归关系。多因素Logistic回归分析结果显示,糖尿病、高血压、冠心病、肥胖及超重是脑梗死的独立危险因素(P﹤0.05),而尚未发现位点rs8050136 A等位基因对脑梗死发病有统计学意义(P﹥0.05)。结论 FTO基因rs8050136多态性可能与汉族人群脑梗死发病无关;FTO基因rs8050136可能是与汉族人群BMI及体质量相关的数量性状位点。

목적:탐토비반역환기인FTO기인위점rs8050136다태성여한족인군뇌경사적상관성。방법선취2011년6월—2012년7월재중남대학상아삼의원신경내과주원적뇌경사환자351례(뇌경사조),령선취동기재본원건강관리중심체검적한족건강인군417례(대조조),수집수시자기본자료,병채집기혈액,응용취합매련반응( PCR)급비행시간질보기술( MALDI-TOF-MS)대FTO기인위점rs8050136진행기인분형검측。결과대조조화뇌경사조성별、년령、체질지수( BMI)、저밀도지단백담고순( LDL-C)、고밀도지단백담고순( HDL-C)、총담고순( TC)비교,차이균무통계학의의( P ﹥0.05);량조당뇨병병사、관심병병사、고혈압병사、체질량、수축압(SBP)、서장압(DBP)、삼선감유(TG)、뇨산(UA)화공복혈당(FBG)비교,차이균유통계학의의(P﹤0.05)。대조조화뇌경사조기인형분포화등위기인빈솔비교,차이균무통계학의의( P﹥0.05)。단인소선성회귀분석결과현시,기인위점순합자AA분별여BMI(b=0.004,b'=0.088,t=2.420,P=0.016)、체질량(b=0.001,b'=0.072, t=1.993,P=0.047)존재선성회귀관계。다인소Logistic회귀분석결과현시,당뇨병、고혈압、관심병、비반급초중시뇌경사적독립위험인소(P﹤0.05),이상미발현위점rs8050136 A등위기인대뇌경사발병유통계학의의(P﹥0.05)。결론 FTO기인rs8050136다태성가능여한족인군뇌경사발병무관;FTO기인rs8050136가능시여한족인군BMI급체질량상관적수량성상위점。
Objective To investigate the correlation between fat mass and obesity-associated gene FTO polymorphism and cerebral infarction in Han people. Methods We enrolled 351 patients who were admitted into the Department of Neurology of the Third Xiangya Hospital of Central South University from June 2011 to July 2012 as cerebral infarction group,and we also enrolled 417 healthy people who received physical examination in the same hospital and period as control group. Basic data of the subjects were collected,and blood samples were taken. PCR and MALDI-TOF-MS were used to detect the genotyping of FTO gene rs8050136. Results Control group and cerebral infarction group were not significantly different(P﹥0. 05)in gender, age,BMI,LDL -C,HDL -C and TC;the two groups were significantly different(P ﹤0. 05)in the medical history of diabetes mellitus,coronary heart disease and hypertension,body mass,SBP,DBP,TG,UA and FBG. The two groups were not significantly different(P﹥0. 05)in genotype distribution and allele frequency. Univariate linear regression analysis showed that homozygote AA of the gene site had linear regression relation with BMI(b=0. 004,b'=0. 088,t=2. 420,P=0. 016) and body mass(b = 0.001,b' = 0.072,t = 1.993,P = 0.047). Multivariate Logistic regression analysis showed that diabetes,hypertension,coronary heart disease,obesity and overweight were independent risk factors for cerebral infarction( P﹤0. 05),while no significant influence of rs8050136 A allele on cerebral infarction was found(P ﹥0. 05). Conclusion rs8050136 polymorphism of FTO may not be associated with cerebral infarction in Han people. rs8050136 of FTO may be a quantitative trait locus associated with BMI and body mass of Han people.
