目的::探讨高危型人乳头瘤病毒感染与核转录因子 NF-кBp50在宫颈上皮瘤变及宫颈鳞癌中的表达及意义。方法:(1)选取52例宫颈鳞癌、67例宫颈上皮瘤变(cervical intraepithelial neoplasia,CIN)组织为研究组,选取20例正常宫颈组织为对照组;(2)采用 Cervista 技术检测宫颈局部 HR-HPV 的感染状态;(3)应用免疫组化 S-P 法检测宫颈标本中 NF-кBp50的表达并以阳性单位(positive unit,PU)表示其阳性反应结果。结果:(1)宫颈上皮瘤变与宫颈鳞癌组中 HR-HPV 的感染率为91.04%与94.23%,高于对照组的15%(P <0.05);(2)NF-кBp50在 CIN 及宫颈鳞癌中表达的 PU 值为(25.42±2.23)与(28.34±3.21),正常宫颈为(6.35±2.16)(P <0.05);(3)在 CINⅠ、CINⅡ及 CINⅢ中 NF-кB p50表达的 PU 值为(21.24±2.53)、(24.56±2.75)与(28.35±3.24)(P <0.05);(4)高、中、低分化宫颈鳞癌中 NF-кBp50表达的 PU 值为(26.87±2.18)、(27.54±3.15)与(29.42±3.56)(P >0.05);(5)Ⅰ、Ⅱ及Ⅲ期宫颈鳞癌中 NF-кBp50阳性表达的 PU 值为(22.43±1.32)、(25.17±3.24)与(27.26±2.52)(P <0.05)。结论:(1)HR-HPV 感染促使 NF-кBp50的过度表达导致宫颈上皮瘤变及宫颈鳞癌的发生,而且 NF-кBp50的早期表达是宫颈癌变发生的早期事件;(2) NF-кBp50的过表达与宫颈鳞癌的病程进展相关,可用于预测其预后。

목적::탐토고위형인유두류병독감염여핵전록인자 NF-кBp50재궁경상피류변급궁경린암중적표체급의의。방법:(1)선취52례궁경린암、67례궁경상피류변(cervical intraepithelial neoplasia,CIN)조직위연구조,선취20례정상궁경조직위대조조;(2)채용 Cervista 기술검측궁경국부 HR-HPV 적감염상태;(3)응용면역조화 S-P 법검측궁경표본중 NF-кBp50적표체병이양성단위(positive unit,PU)표시기양성반응결과。결과:(1)궁경상피류변여궁경린암조중 HR-HPV 적감염솔위91.04%여94.23%,고우대조조적15%(P <0.05);(2)NF-кBp50재 CIN 급궁경린암중표체적 PU 치위(25.42±2.23)여(28.34±3.21),정상궁경위(6.35±2.16)(P <0.05);(3)재 CINⅠ、CINⅡ급 CINⅢ중 NF-кB p50표체적 PU 치위(21.24±2.53)、(24.56±2.75)여(28.35±3.24)(P <0.05);(4)고、중、저분화궁경린암중 NF-кBp50표체적 PU 치위(26.87±2.18)、(27.54±3.15)여(29.42±3.56)(P >0.05);(5)Ⅰ、Ⅱ급Ⅲ기궁경린암중 NF-кBp50양성표체적 PU 치위(22.43±1.32)、(25.17±3.24)여(27.26±2.52)(P <0.05)。결론:(1)HR-HPV 감염촉사 NF-кBp50적과도표체도치궁경상피류변급궁경린암적발생,이차 NF-кBp50적조기표체시궁경암변발생적조기사건;(2) NF-кBp50적과표체여궁경린암적병정진전상관,가용우예측기예후。
Objective:To investigate the infection of HR-HPV and the expression of nuclear transcription factor NF-кBp50 in CIN and quamous carcinoma of the cervix and its significance.Methods:(1)Select 52 cases of quamous carcinoma of the cervix and 67 cases of CIN as the research group,select 20 cases with normal cervix as the control group;(2)Detect the infection of HR-HPV on part of cervix with Cervista technology;(3)Detect the expression of NF-кBp50 in the sample of cervix with S-P immunohistochemistry and denote its positive reaction results in positive units.Results:(1)The infection rate of HR-HPV in the group of CIN and squamous carcinoma of the cervix was 91.04% and 94.23%,which was higher than that of the control group (15%),P <0.05;(2)The value of PU expressed by NF-кBp50 in CIN and squamous carcinoma of the cervix was 25.42±2.23 and 28.34±3.21 respectively,while that of cervix was 6.35±2.1 6,P <0.05;(3)The PU val-ue expressed by NF-кB p50 in CINⅠ,CINⅡ and CINⅢ was 21.24±2.53,24.56 ±2.75,24.56 ±2.75 respec-tively,P <0.05;(4)The PU value expressed by NF-кBp50 in highly,moderately and poorly differentiated squamous carcinoma of the cervix was 26.87±2.18,27.54±3.15 and 29.42 ±3.56 respectively P <0.05;(5) The PU value expressed by NF-кBp50 in squamous carcinoma of the cervix in phrase I ,Ⅱ and Ⅲ were 22.43 ±1.32,25.17±3.24 and 27.26 ±2.52,P <0.05.Conclusion:(1 )The infection of HR-HPV boost the overex-pression of NF-кBp50 and results in CIN and squamous carcinoma of the cervix,and the expression of NF-кBp50 in early stage is the event of uterine cervix cancer in early stage;(2)The over expression of NF-кBp50 is related to the development of the course of squamous carcinoma of the cervix,which can be applied to pre-dict its prognosis.
