Diabetic patients with renal microvascular disease%glycosylated hemoglobin%homocysteine%urinary albu-min
目的:探讨糖化血红蛋白、同型半胱氨酸及尿微量白蛋白与2型糖尿病肾脏微血管病变的关系。方法选择95例2型糖尿病患者作为观察组研究对象,同期来医院参加体检的健康志愿者作为对照组,检测研究对象的糖化血红蛋白、同型半胱氨酸和尿微量白蛋白及血生化指标,分析比较各组糖化血红蛋白、同型半胱氨酸及尿微量白蛋白水平。结果糖尿病合并肾脏微血管病变组研究对象的糖化血红蛋白、同型半胱氨酸及尿微量白蛋白水平高于单纯糖尿病组患者和对照组研究对象,同型半胱氨酸随血糖升高、肾脏损害程度的加深而升高,差异有统计学意义(P <0.05)。糖化血红蛋白水平升高,空腹血糖和尿微量白蛋白水平也随之增高。多因素 Logistic 回归分析显示糖化血红蛋白、同型半胱氨酸和尿微量白蛋白与糖尿病发生肾脏微血管病变呈正相关(OR 值分别为1.27、1.41和1.53)。结论2型糖尿病合并肾脏微血管病变患者的糖化血红蛋白、同型半胱氨酸及尿微量白蛋白水平显著升高,临床检测糖尿病患者的糖化血红蛋白、同型半胱氨酸及尿微量白蛋白对预防糖尿病合并肾脏微血管病变有重要意义。

목적:탐토당화혈홍단백、동형반광안산급뇨미량백단백여2형당뇨병신장미혈관병변적관계。방법선택95례2형당뇨병환자작위관찰조연구대상,동기래의원삼가체검적건강지원자작위대조조,검측연구대상적당화혈홍단백、동형반광안산화뇨미량백단백급혈생화지표,분석비교각조당화혈홍단백、동형반광안산급뇨미량백단백수평。결과당뇨병합병신장미혈관병변조연구대상적당화혈홍단백、동형반광안산급뇨미량백단백수평고우단순당뇨병조환자화대조조연구대상,동형반광안산수혈당승고、신장손해정도적가심이승고,차이유통계학의의(P <0.05)。당화혈홍단백수평승고,공복혈당화뇨미량백단백수평야수지증고。다인소 Logistic 회귀분석현시당화혈홍단백、동형반광안산화뇨미량백단백여당뇨병발생신장미혈관병변정정상관(OR 치분별위1.27、1.41화1.53)。결론2형당뇨병합병신장미혈관병변환자적당화혈홍단백、동형반광안산급뇨미량백단백수평현저승고,림상검측당뇨병환자적당화혈홍단백、동형반광안산급뇨미량백단백대예방당뇨병합병신장미혈관병변유중요의의。
Objective To investigate the relationship between glycosylated hemoglobin,homocysteine and urinary al-bumin and renal microvascular disease in type 2 diabetic nephropathy.Methods 95 patients with type 2 diabetes were selected as the study group of the observation group,the healthy volunteers who participated in physical examination during the same period served as the control group.Glycosylated hemoglobin,homocysteine,urine albumin and blood bi-ochemical index of the subjects were measured,the levels of glycosylated hemoglobin,homocysteine and urinary albumin in the groups were compared.Results The levels of glycosylated hemoglobin,Hcy and Urinary albumin in diabetic pa-tients with renal microvascular disease group were higher than those of the diabetic group and the control group.Homo-cysteine increased with the increase of blood glucose and renal damage degree,the difference was statistically significant (P <0.05).The levels of glycosylated hemoglobin were elevated,and the levels of fasting blood glucose and urine albu-min were also increased.Multivariate logistic regression analysis showed that glycated hemoglobin,with cysteine and u-rine albumin and diabetes occurrence of renal micro vascular lesions were positively related (OR = 1.27,1.41 and 1.53).Conclusion The levels of glycosylated hemoglobin,homocysteine and urinary albumin were significantly in-creased in type 2 diabetic patients with renal microvascular disease,the clinical detection of glycosylated hemoglobin,ho-mocysteine and urinary albumin in patients with diabetes mellitus is important for preventing diabetic renal microvascu-lar disease.
