目的:探讨基质金属蛋白酶抑制基因(reversion-inducing cysteinerich protein with Kazal Motifs,RECK)、三叶因子2(Trefoil factors2,TFF2)、雌激素受体β亚型(ERβ)在子宫内膜癌中的表达及抑癌基因 p16甲基化的临床意义。方法随机收集子宫内膜癌术后组织标本80例与同期术后正常子宫内膜组织标本45例,采用免疫组化法检测 RECK、TFF2与 ERβ的蛋白表达,MSP 法检测 p16甲基化。结果RECK、TFF2、ERβ在子宫内膜癌组织中的阳性表达率分别为:50.00%、56.25%与48.75%,均显著低于正常组(82.22%、86.67%与80.00%),差异具有统计学意义(P <0.05);子宫内膜癌组 TFF2、ERβ的阳性表达率均显著高于宫颈癌组(P <0.05),而子宫内膜癌组 RECK、TFF2、ERβ的阳性表达率与术前化疗癌症组比较无显著性差异(均 P >0.05)。子宫内膜癌组织中 RECK 、TFF2的阳性表达与临床分期、组织学分级、肌层浸润程度及淋巴结转移密切相关;ERβ的表达与组织学分级、肌层浸润程度及淋巴结转移密切相关,而与临床分期无相关性,子宫内膜癌组织存在 p16基因全部或部分甲基化,正常子宫内膜组织不存在 p16基因甲基化现象;其中子宫内膜癌组织中 p16甲基化有42例(52.50%),p16甲基化与临床分期,组织学分级、肌层浸润与淋巴结转移密切相关。结论RECK、TFF2、ERβ、p16甲基化在子宫内膜癌的不同组织学分级、肌层浸润程度与有无淋巴结转移中的表达各异,并在子宫内膜癌的诊治中具有临床应用价值。

목적:탐토기질금속단백매억제기인(reversion-inducing cysteinerich protein with Kazal Motifs,RECK)、삼협인자2(Trefoil factors2,TFF2)、자격소수체β아형(ERβ)재자궁내막암중적표체급억암기인 p16갑기화적림상의의。방법수궤수집자궁내막암술후조직표본80례여동기술후정상자궁내막조직표본45례,채용면역조화법검측 RECK、TFF2여 ERβ적단백표체,MSP 법검측 p16갑기화。결과RECK、TFF2、ERβ재자궁내막암조직중적양성표체솔분별위:50.00%、56.25%여48.75%,균현저저우정상조(82.22%、86.67%여80.00%),차이구유통계학의의(P <0.05);자궁내막암조 TFF2、ERβ적양성표체솔균현저고우궁경암조(P <0.05),이자궁내막암조 RECK、TFF2、ERβ적양성표체솔여술전화료암증조비교무현저성차이(균 P >0.05)。자궁내막암조직중 RECK 、TFF2적양성표체여림상분기、조직학분급、기층침윤정도급림파결전이밀절상관;ERβ적표체여조직학분급、기층침윤정도급림파결전이밀절상관,이여림상분기무상관성,자궁내막암조직존재 p16기인전부혹부분갑기화,정상자궁내막조직불존재 p16기인갑기화현상;기중자궁내막암조직중 p16갑기화유42례(52.50%),p16갑기화여림상분기,조직학분급、기층침윤여림파결전이밀절상관。결론RECK、TFF2、ERβ、p16갑기화재자궁내막암적불동조직학분급、기층침윤정도여유무림파결전이중적표체각이,병재자궁내막암적진치중구유림상응용개치。
Objective To study the expression of RECK,TFF2,ERβ in endometrial carcinoma and clinical signifi-cance of p16 methylation.Methods 80 cases of endometrial carcinoma and 45 cases of normal endometrium tissues were randomly collected,using immunohistochemical method to detect the protein expression of RECK,TFF2 and ERβ,MSP method to detect p16 methylation.Results The positive expression rate of RECK,TFF2,ERβin endometrial carcinoma tissue are respectively 50.00%,56.25% and 48.75%,significantly lower than normal group (82.22%,86.67% and 80.00%)(P <0.05),the positive expression rate of TFF2,ERβin endometrial carcinoma tissue are significantly higher than that of cervical cancer (P <0.05),while compared with preoperative chemotherapy in cancer group,the positive expression rate of RECK,TFF2,ERβin endometrial carcinoma tissue are there are no significant difference (P >0.05). The positive expression of RECK,TFF2 in endometrial carcinoma tissues are closely related to clinical stage,histologi-cal grading,muscular layer the degree of infiltration and lymph node metastasis;The expression of ERβis closely relat-ed to degree of histological grading,muscular invasion and lymph node metastasis,and no correlation with the clinical stages,p16 gene of endometrial tissue are whole or part methylation,normal endometrium tissues have not p16 gene methylation;42 cases endometrial carcinoma tissues are p16 methylation (52.50%)and p16 methylation are closely re-lated to the clinical stage,histological grading,muscular invasion and lymph node metastasis.Conclusion The different expression of RECK,TFF2,ERβ,p16 methylation of endometrial carcinoma in different histological grading,muscular infiltration degree and lymph node metastasis,it is of clinical application value in the diagnosis and treatment of endome-trial carcinoma.
