目的：：对腰椎穿刺联合头颅 CT 在诊断自发性蛛网膜下腔出血上的临床效果进行分析和总结。方法：选取医院在2011年3月~2014年3月确诊为自发性蛛网膜下腔出血的患者72例,对患者的临床表现、腰穿和头颅 CT 检查的结果进行分析。结果：经头颅 CT 检查发现58例患者确诊为自发性蛛网膜下腔出血症状,阳性确诊率为80.56%,后联合腰椎穿刺术进行诊断,72例患者均得以确诊,阳性确诊率为100%。经比较发现,头颅 CT 后联合腰椎穿刺的确诊率明显优于单纯采取头颅 CT,差异比较(P <0.05),有统计学意义。结论：在临床自发性蛛网膜下腔出血的诊断中,单纯的采取腰穿或头颅 CT 检查各有利弊,而采取腰椎穿刺联合头颅CT 相结合的诊断方法,能够有效的提高诊断的正确性,减少误诊的发生,具有重要的临床诊断价值。

Objective:To analyze and summarize the clinical effect of lumbar puncture combined with CT on brain in the diagnosis of spontaneous subarachnoid hemorrhage.Methods:Select 72 patients diagnosed with spontaneous subarachnoid hemorrhage in hospital during the period from March 201 1 to 2014 March , analyze patients'clinical features and the Result of lumbar puncture and CT.Results:According to CT scan, 58 cases were conformed with spontaneous subarachnoid hemorrhage.The positive rate was 80.56%,and 72 cases were all confirmed with the combination lumbar puncture.The positive rate was 100%.Through com-parison It was found that the rate of the conformation of spontaneous subarachnoid hemorrhage according to CT on brain and lumbar puncture was significantly higher than that in virtue of CT alone,and the difference was statistically significant (CT).Conclusion:Clinically,in the diagnosis of spontaneous subarachnoid hem-orrhage,lumbar puncture and CT have their own advantages and disadvantages,the combination of lumbar puncture and CT ,can effectively improve diagnosis accuracy and reduce misdiagnosis,it is of important val-ue in clinical diagnosis.
