目的:探讨儿童脑膜炎患者血清降钙素原(PCT)水平的变化及其与 C-反应蛋白(CRP)的关系。方法检测44例脑膜炎患儿血清 PCT 水平,其中化脓性脑膜炎患儿12例,病毒性脑膜炎患儿32例,同时测定其血 CRP、白细胞(WBC)及脑脊液相关指标[蛋白、WBC、氯化物及葡萄糖],并做统计学分析。结果化脓性脑膜炎组血清 PCT 水平为(9.30±20.47)μg/L,明显高于病毒性脑膜炎组[(0.37±0.27)μg/L](P <0.01);脑膜炎患儿血清 PCT 与 CRP、脑脊液蛋白、WBC 呈明显正相关[相关系数(r)分别为0.343、0.437、0.439,P 均<0.05),与氯化物呈负相关(r =-0.387,P <0.01);进一步多元回归分析显示,血 CRP(β=0.648,P <0.01)影响了40.5%的 PCT 变化,且 PCT 也是脑膜炎患儿 CRP 水平的决定因素之一。结论化脓性脑膜炎患儿血清 PCT 水平明显升高。升高的 PCT 与 CRP 的增加有一定关联,可能通过参与细菌感染诱导的炎症反应促进化脓性脑膜炎的发生、发展。

목적:탐토인동뇌막염환자혈청강개소원(PCT)수평적변화급기여 C-반응단백(CRP)적관계。방법검측44례뇌막염환인혈청 PCT 수평,기중화농성뇌막염환인12례,병독성뇌막염환인32례,동시측정기혈 CRP、백세포(WBC)급뇌척액상관지표[단백、WBC、록화물급포도당],병주통계학분석。결과화농성뇌막염조혈청 PCT 수평위(9.30±20.47)μg/L,명현고우병독성뇌막염조[(0.37±0.27)μg/L](P <0.01);뇌막염환인혈청 PCT 여 CRP、뇌척액단백、WBC 정명현정상관[상관계수(r)분별위0.343、0.437、0.439,P 균<0.05),여록화물정부상관(r =-0.387,P <0.01);진일보다원회귀분석현시,혈 CRP(β=0.648,P <0.01)영향료40.5%적 PCT 변화,차 PCT 야시뇌막염환인 CRP 수평적결정인소지일。결론화농성뇌막염환인혈청 PCT 수평명현승고。승고적 PCT 여 CRP 적증가유일정관련,가능통과삼여세균감염유도적염증반응촉진화농성뇌막염적발생、발전。
Objective To investigate the relationship between the level change of serum procalcitonin (PCT)and C-reactive protein (CRP)in children with meningitis.Methods The levels of serum PCT in 41 children with purulent(n=12)or viral(n=32)meningitis were determined,the levels of CRP,white blood cell (WBC)and cerebrospinal fluid related indicators[protein,WBC,chloride and glucose]were determined simultaneously,and the results were analyzed statistically.Results The PCT levels in purulent meningitis group [(9.30± 20.47)μg/L]were significantly higher than those in viral meningitis group [(0.37 ±0.27)μg/L](P <0.01).The PCT levels in children with meningitis were positively correlated with CRP,cerebrospinal fluid protein and WBC[correlation coefficient (r)= 0.343,0.437 and 0.439,P <0.05],and negatively with chloride (r=-0.387,P <0.01).Multiple linear regression analysis found that the change of CRP(β=0.648,P <0.01)influenced the 40.5% of the change of PCT.In addition,PCT was one of factors for the CRP levels in children with meningitis.Conclusion The levels of serum PCT in 41 children with puru-lent meningitis increase significantly,which are associated with the increase of CRP,promoting the occurrence and de-velopment of purulent meningitis by participating in the bacterial infection-induced inflammatory response.
