目的:探讨血清铁蛋白(SF)和胚胎性癌抗原(CEA)、细胞角蛋白19片段抗原21-1(CYFRA21-1)、神经特异性烯醇化酶(NSE)在肺癌诊断中的应用价值。方法选取2015年3月~2015年6月100例肺癌患者和30例健康对照者,采用罗氏电化学发光免疫分析仪检测患者血清中SF和CEA,CYFRA21-1,NSE的含量。结果肺癌组SF,CEA,CYFRA21-1,NSE的含量分别为(456.39±612.43)μg/L,(45.94±135.20)μg/L,(15.02±38.60)μg/L,(35.61±60.55)μg/L,显著高于健康对照组(171.15±79.18)μg/L,(3.26±2.22)μg/L,(1.92±0.72)μg/L,(13.47±3.45μg/L),差异有统计学意义(P<0.01)。 SF与CEA,CYFRA21-1,NSE联合检测比CEA,CYFRA21-1,NSE 3项联合及各单项检测能够提高肺癌诊断的敏感度(92%>90%>78%>54%>52%>22%);肺癌组SF的水平与CEA,YFRA21-1,NSE的水平没有显著的相关性( r=0.186,P=0.196>0.05;r=0.234,P=0.101>0.05;r=0.316,P=0.025<0.05)。结论 SF能够提高肺癌检测的敏感度,可以将SF作为一种辅助的肿瘤血清标志物用于肺癌的早期诊断中。

목적:탐토혈청철단백(SF)화배태성암항원(CEA)、세포각단백19편단항원21-1(CYFRA21-1)、신경특이성희순화매(NSE)재폐암진단중적응용개치。방법선취2015년3월~2015년6월100례폐암환자화30례건강대조자,채용라씨전화학발광면역분석의검측환자혈청중SF화CEA,CYFRA21-1,NSE적함량。결과폐암조SF,CEA,CYFRA21-1,NSE적함량분별위(456.39±612.43)μg/L,(45.94±135.20)μg/L,(15.02±38.60)μg/L,(35.61±60.55)μg/L,현저고우건강대조조(171.15±79.18)μg/L,(3.26±2.22)μg/L,(1.92±0.72)μg/L,(13.47±3.45μg/L),차이유통계학의의(P<0.01)。 SF여CEA,CYFRA21-1,NSE연합검측비CEA,CYFRA21-1,NSE 3항연합급각단항검측능구제고폐암진단적민감도(92%>90%>78%>54%>52%>22%);폐암조SF적수평여CEA,YFRA21-1,NSE적수평몰유현저적상관성( r=0.186,P=0.196>0.05;r=0.234,P=0.101>0.05;r=0.316,P=0.025<0.05)。결론 SF능구제고폐암검측적민감도,가이장SF작위일충보조적종류혈청표지물용우폐암적조기진단중。
Objective To analyze the diagnostic value of serum ferritin(SF) and Carcinoembryonic antigen(CEA),Cytokeratin 19 fragment antigen 21-1(CYFRA21-1),Neural specificity enolization enzyme(NSE) in lung cancer.Methods SF level and markers of CEA,CYFRA21-1,NSE in peripheral blood were examined by Roche electrochemiluminescence of 100 lung cancer patients and 30 healthy controls.Results The level of SF in patients was higher than that of healthy controls[(456.39 ±612.43)μg/L vs (171.15 ±79.18)μg/L, t=3.134,P=0.003].The level of CEA in patients was higher than that of healthy controls[(45.94 ±135.20)μg/L vs (3.26 ±2.22)μg/L,t=3.514,P=0.002].The level of CYFRA21-1 in patients was higher than that of healthy controls[(15.02 ±38.60)μg/L vs (1.92 ± 0.72)μg/L,t=3.502,P=0.001].The level of NSE in patients was higher than that of healthy controls[(35.61 ±60.55)μg/L vs (13.47 ±3.45)μg/L,t=3.614,P=0.000].The SF and three kinds of tumor markers joint detection sensitivity were higher than that of each target detection alone and three kinds of tumor markers joint(92%>90%>78%>54%>52%>22%).The SF level was not significantly corre-lation with level of CEA,CYFRA21-1,NSE(r=0.186,P=0.196>0.05;r=0.234,P=0.101>0.05;r=0.316,P=0.025<0.05).Con-clusion The serum SF can improve the detection sensitivity,so the level of SF can become an assistant tumor marker used in lung cancer early diagnosis.
