目的:探讨参黄散对术后肠动力障碍大鼠胃肠功能、血清Ghrelin水平及其平滑肌受体表达的影响。方法:将SD雄性大鼠随机分为假手术组、模型组、参黄散组, GHSR拮抗剂组,每组再随机分为造模后6、12、24、48、72 h和1周组,每组6只。GHSR拮抗剂组于造模前30 min以1μmol·kg-1剂量给予[D-Lys3]-GHRP-6腹腔注射。参黄散组和GHSR拮抗剂组造模后参黄散外敷大鼠的脾俞和肾俞,而假手术组(仅行剖腹探查术)和模型组造模后使用等量细沙包外敷脾俞和肾俞。各组于造模后6、12、24、48、72 h和1周时间点观察大鼠胃肠道传输几何均数和血清Ghrelin水平,另取造模后72 h各组大鼠10 cm上段空肠测定小肠平滑肌细胞Ghrelin受体(GHS-R1a)的表达。结果:与假手术组相比,模型组血清Ghrelin水平和胃肠道传输几何均数在造模后6、12、24、48和72 h时间点均降低(P<0.05,P<0.01);在24、48、72 h时间点参黄散组较模型组血清Ghrelin水平和胃肠道传输几何均数明显增加(P<0.05,P<0.01),较GHSR拮抗剂组胃肠道传输几何均数明显增加(P<0.01);在12、24、48和72 h时间点参黄散组血清Ghrelin水平与胃肠道传输几何均数的变化呈显著性正相关,r<1(P<0.01)。造模后72 h,模型组、GHSR拮抗剂组、参黄组和假手术组小肠平滑肌细胞GHS-R1a表达量逐渐增多。结论:参黄散具有显著促进术后肠动力障碍大鼠胃肠道传输功能,其作用机制可能与促进Ghrelin分泌和增加GHS-R1a表达有关。

목적:탐토삼황산대술후장동력장애대서위장공능、혈청Ghrelin수평급기평활기수체표체적영향。방법:장SD웅성대서수궤분위가수술조、모형조、삼황산조, GHSR길항제조,매조재수궤분위조모후6、12、24、48、72 h화1주조,매조6지。GHSR길항제조우조모전30 min이1μmol·kg-1제량급여[D-Lys3]-GHRP-6복강주사。삼황산조화GHSR길항제조조모후삼황산외부대서적비유화신유,이가수술조(부행부복탐사술)화모형조조모후사용등량세사포외부비유화신유。각조우조모후6、12、24、48、72 h화1주시간점관찰대서위장도전수궤하균수화혈청Ghrelin수평,령취조모후72 h각조대서10 cm상단공장측정소장평활기세포Ghrelin수체(GHS-R1a)적표체。결과:여가수술조상비,모형조혈청Ghrelin수평화위장도전수궤하균수재조모후6、12、24、48화72 h시간점균강저(P<0.05,P<0.01);재24、48、72 h시간점삼황산조교모형조혈청Ghrelin수평화위장도전수궤하균수명현증가(P<0.05,P<0.01),교GHSR길항제조위장도전수궤하균수명현증가(P<0.01);재12、24、48화72 h시간점삼황산조혈청Ghrelin수평여위장도전수궤하균수적변화정현저성정상관,r<1(P<0.01)。조모후72 h,모형조、GHSR길항제조、삼황조화가수술조소장평활기세포GHS-R1a표체량축점증다。결론:삼황산구유현저촉진술후장동력장애대서위장도전수공능,기작용궤제가능여촉진Ghrelin분비화증가GHS-R1a표체유관。
Objective:To investigate the effect of Senhuang powder(SHP) on gastrointestinal transit,serum ghrelin and its receptor expression in rat model of postoperative ileus(POI).Methods:SD rats were randomly divided into sham operation group (SHAM), intestinal manipulation group (IM), Senhuang powder group (SHP) and GHSR antagonist group (GHSRA).The POI models were established in IM,SHP and GHSRA groups. The SHAM group was operated with exploratory laparotomy only. After the model was successfully established, SHP and GHSRA groups were transdermally administered with SHP on the animals’Pishu points and Shenshu points while SHAM and IM groups with equal volume of fine sandbag. Serum ghrelin level and geometric centre(GC) of gastrointestinal transit parameter were determined from each group at 6h,12h,24h,48h,72h and 1w postsurgery respectively. Meanwhile, the expression of ghrelin receptors (GHS-Rla) in small intestine muscle layers was examined by Western blot at 72h after surgery. Results:At 6h,12h,24h,48h and 72h after surgery,serum ghrelin level and GC in IM were dramatically lower than those in SHAM (P<0.05,P<0.01).At 24h,48h and 72h, Serum ghrelin level and GC of SHP increased significantly,compared with the IM and the GC of SHP were also higher than that of GHSRA (P<0.05,P<0.01). at 12h,24h,48h and 72h GC of SHP were correlated positively with serum ghrelin level,r<1(P<0.01).The GHS-R1a express of intestine muscularis in IM,GHSRA,SHP and SHAM increased gradually.Conclusion:SHP can promote the recuperation of enterokinesia in rat model of POI through increasing the levels of serum ghrelin and expression of GHS-R1a in intestinal muscularis.
