目的：探讨75岁以上老人轻微外伤性硬膜下血肿患者的合理治疗方法。方法回顾性收集1990年1月~2014年11月海军总医院治疗的硬膜下血肿患者的临床资料97例。根据患者的处置方式，分为钻孔引流手术组和保守治疗组，分析治疗后1周时两组患者的临床疗效和认知功能改善。结果有手术指征的急性硬膜下血肿，1周时钻孔引流组的临床缓解率高于保守治疗组，1周时钻孔引流组的认知功能恢复优于保守治疗组。结论75岁以上轻微脑外伤导致的硬膜下血肿，一旦具有手术指征，应首选钻孔引流术治疗。
목적：탐토75세이상노인경미외상성경막하혈종환자적합리치료방법。방법회고성수집1990년1월~2014년11월해군총의원치료적경막하혈종환자적림상자료97례。근거환자적처치방식，분위찬공인류수술조화보수치료조，분석치료후1주시량조환자적림상료효화인지공능개선。결과유수술지정적급성경막하혈종，1주시찬공인류조적림상완해솔고우보수치료조，1주시찬공인류조적인지공능회복우우보수치료조。결론75세이상경미뇌외상도치적경막하혈종，일단구유수술지정，응수선찬공인류술치료。
Objective To investigate the effects of burr-hole trepanation for subdural hematoma caused by mild trarmatic brain injury in over-75-year old patients. Methods A retrospective analysis was performed on a total of 97 subdural hematoma patients who were admitted at the hospital from January 1990 to November 2014. The patients who received drainage surgery were selected into surgical group while the others were selected into the conservative treatment group. The analysis included the clinical result and their cognitive function changes one week after the surgery. Results The relief rate of surgery group was higher than the conservative treatment group. The cognitive function change of the surgery group was better than the conservative treatment group. Conclusion Burr-hole trepanation is the first choice for subdural hematoma caused by mild trarmatic brain injury in over 75-year olds.
