目的：探讨超声引导下腋路臂丛神经阻滞在自体动静脉内瘘成形手术中的麻醉效果及安全性。方法选择拟实施自体动静脉内瘘成形手术的患者40例，随机均分为盲探法腋路臂丛阻滞组（A 组）及超声引导下腋路臂丛阻滞组（B 组），每组20例，两组均给予1%氯普鲁卡因注射液与0.375%罗哌卡因注射液的混合液30 ml；观察患者的阻滞完成时间、阻滞持续时间及术中的麻醉效果。结果 B 组患者的神经阻滞完成时间较 A 组短，阻滞持续时间较 A 组长，麻醉效果优于 A 组，差异有统计学意义（P <0.05）。结论超声引导下腋路臂丛神经阻滞起效较快，维持时间较长，麻醉效果确切，更适用于自体动静脉内瘘成形手术。
목적：탐토초성인도하액로비총신경조체재자체동정맥내루성형수술중적마취효과급안전성。방법선택의실시자체동정맥내루성형수술적환자40례，수궤균분위맹탐법액로비총조체조（A 조）급초성인도하액로비총조체조（B 조），매조20례，량조균급여1%록보로잡인주사액여0.375%라고잡인주사액적혼합액30 ml；관찰환자적조체완성시간、조체지속시간급술중적마취효과。결과 B 조환자적신경조체완성시간교 A 조단，조체지속시간교 A 조장，마취효과우우 A 조，차이유통계학의의（P <0.05）。결론초성인도하액로비총신경조체기효교쾌，유지시간교장，마취효과학절，경괄용우자체동정맥내루성형수술。
Objective:To assess the efficacy and safety of ultrasound-guided axillary brachial plexus block in patients undergoing arteriovenous fistula angioplasty. Methods:Forty patients accepted arteriovenous fistula angioplasty were equally randomized into 2 groups,anatomic localization for axillary brachial plexus block(Group A)and ultrasound-guided axillary brachial plexus block(Group B). The mixture of 1% chloroprocaine and 0. 375% ropivacaine was injected in the two groups,with 30ml respectively. The block completion time,the block duration and the anesthetic effect were compared be-tween the two groups. Results:Compared with Group A,the block completion time was shorter,the block duration was longer,and the anesthetic effect was superior in Group B(P < 0. 05). Conclusion:The ultrasound-guided axillary brachial plexus block applies to patients undergoing arteriovenous fistula angioplasty,with the advantages of rapid onset,long dura-tion and perfect anesthetic effect.
