目的：统计胫骨平台骨折术后并发症的发生率，探索对应的防治措施。方法计算机检索 Medline、Em-base、中国生物医学文献数据库、万方数据库和中国知网等，检索文献发表时间为2001年1月至2014年5月，获取胫骨平台骨折术后并发症发生率的相关文献。通过 R 软件 Meta 软件包中的 Metaprop 函数进行统计分析，记录胫骨平台骨折术后常见并发症的发生率。结果共纳入24篇文献，其中前瞻性研究9篇，回顾性分析15篇。常见并发症包括感染、深静脉血栓形成、复位不良、关节僵直、关节炎，其发生率分别为6.3％[95％ CI（0.04；0.09）]、2.5％[95％ CI （0.02；0.04）]、9.3％[95％ CI（0.06；0.14）]、4.4％[95％ CI（0.03；0.08）]、10.2％[95％ CI（0.07；0.15）]。结论感染、深静脉血栓形成、复位不良、关节僵直及关节炎等胫骨平台骨折术后常见并发症的发生率较高，对患者预后影响显著，围手术期需积极采取预防措施以降低手术风险。
목적：통계경골평태골절술후병발증적발생솔，탐색대응적방치조시。방법계산궤검색 Medline、Em-base、중국생물의학문헌수거고、만방수거고화중국지망등，검색문헌발표시간위2001년1월지2014년5월，획취경골평태골절술후병발증발생솔적상관문헌。통과 R 연건 Meta 연건포중적 Metaprop 함수진행통계분석，기록경골평태골절술후상견병발증적발생솔。결과공납입24편문헌，기중전첨성연구9편，회고성분석15편。상견병발증포괄감염、심정맥혈전형성、복위불량、관절강직、관절염，기발생솔분별위6.3％[95％ CI（0.04；0.09）]、2.5％[95％ CI （0.02；0.04）]、9.3％[95％ CI（0.06；0.14）]、4.4％[95％ CI（0.03；0.08）]、10.2％[95％ CI（0.07；0.15）]。결론감염、심정맥혈전형성、복위불량、관절강직급관절염등경골평태골절술후상견병발증적발생솔교고，대환자예후영향현저，위수술기수적겁채취예방조시이강저수술풍험。
Objective To analyze the complications following the operative treatment of tibial plateau fracture through a meta-analysis. Methods A comprehensive search was conducted for all articles addressing the postoperative complications of tibial plateau fracture published in English and Chinese from January 2001 to May 2015. A proportion meta-analysis across the stud-ies was performed for the complications after operation with the use of R software. Results 24 studies,9 prospective and 15 retrospective ones,were identified. The common complications included infection,deep venous thrombosis,malreduction,joint stiffness,osteoarthritis. Their incidences were respectively 6. 3% ,[95% CI(0. 04;0. 09)];2. 5% ,[95% CI(0. 02;0. 04)];9. 3% ,[95% CI(0. 06;0. 14)];4. 4% ,[95% CI(0. 03;0. 08)];10. 2% ,[95% CI(0. 07;0. 15)]. Conclusion Since the incidences of complications like infection,deep venous thrombosis,malreduction,joint stiffness and osteoarthritis are high,they can directly affect the outcomes of the operative treatment of tibial plateau fractures. Surgeons should take active prophylactic measures during the perioperative period to reduce the operative risks.
