目的 对比顺行和逆行球囊扩张加钬激光内切开术治疗输尿管上段狭窄的临床效果.方法 回顾分析本院2008年1月至2014年6月,对54例采用顺行和逆行球囊扩张术治疗输尿管上段狭窄54例病例资料并进行随访.结果 54例手术均一期顺利完成,两组治愈率(顺行组/逆行组:18/30vs13/24min)、手术时间(顺行组/逆行组:39.5±4.3 vs36.2±6.4min)无明显差异,顺行组血红蛋白下降情况(5.4±4.6vs1.8±3.3g/L)、并发症(8/30vs3/24)、住院时间(8.7±3.5vs3.5±2.7d)、住院费用(2.05±0.19vs1.77±0.30万元)明显高于逆行组(P＜0.05).结论 顺行和逆行球囊扩张加钬激光内切开术治疗输尿管上段狭窄均为安全、有效、微创的方法,两组疗效相当,临床上各有各的优势.

Objectives To assess and compare the security,efficacy of anterograde and retrograde balloon dilatation combined with holmium laser in the treatment of upper-ureteral strictures.Methods Between January 2008 and June 2014,54 cases of upper-ureteral strictures treated with anterograde and retrograde balloon dilatation combined with holmium laser in our hospital were retrospectively reviewed.Results The procedures were succeed in all patients.The results showed that retrograde balloon dilatation was better than antcrograde balloon dilatation in hemoglin drop (1.8 ± 3.3 vs5.4 ± 4.6 g/L),mean hospital stay (3.5 ± 2.7 vs8.7 ± 3.5 d),complications (3/24 vs8/30),the cost of the treatment (1.77 ±0.30vs2.05 ± 0.19million yuan).There was no significant difference between the anterograde balloon dilatation group and retrograde balloon dilatation group in mean operating time (39.5 ± 4.3vs36.2 ±6.4min),healing rate(18/30vs13/24).Conclusions Anterograde and retrograde balloon dilatation combined with holmium laser are safe,effective and mini-invasive treatment for upper-ureteral strictures.There are no difference in the success rate,each have there own advantages.
