目的:分析 PET 和脑磁图在术前评估 MRI 阴性癫痫中的作用。方法选取12例难治性癫痫(TLE)患者为研究对象,比较患者术前 PET 和脑磁图与手术结果。同时选取15名正常健康志愿者,通过病史、一般体格检查和常规实验室检测进行筛选。结果①所有纳入本研究患者大脑 MRI 被视为没有 MRI 阴性癫痫的诊断价值。12例患者完全是正常的(1例在前叶显示极微小的异常)。②18 F - FCWAY 的血浆游离分数(f1)在 TLE 患者高于健康对照(0.13±0.04 vs.0.09±0.07,P ﹤0.10)。③在局灶化 MEG 刺激区组8例,所有8例有局灶化癫痫发作。单侧性的 MEG 刺激区组7例,3例局灶化癫痫发作。非单侧 MEG 刺激区组2例没有局灶化癫痫发作。结论术前定位 MRI 阴性癫痫,PET/ MEG 结果一致时,手术结果判定比较准确。

목적:분석 PET 화뇌자도재술전평고 MRI 음성전간중적작용。방법선취12례난치성전간(TLE)환자위연구대상,비교환자술전 PET 화뇌자도여수술결과。동시선취15명정상건강지원자,통과병사、일반체격검사화상규실험실검측진행사선。결과①소유납입본연구환자대뇌 MRI 피시위몰유 MRI 음성전간적진단개치。12례환자완전시정상적(1례재전협현시겁미소적이상)。②18 F - FCWAY 적혈장유리분수(f1)재 TLE 환자고우건강대조(0.13±0.04 vs.0.09±0.07,P ﹤0.10)。③재국조화 MEG 자격구조8례,소유8례유국조화전간발작。단측성적 MEG 자격구조7례,3례국조화전간발작。비단측 MEG 자격구조2례몰유국조화전간발작。결론술전정위 MRI 음성전간,PET/ MEG 결과일치시,수술결과판정비교준학。
Objective To analyze the effect of PET and magnetic encephalography in evaluating MRI negative epilepsy. Methods 12 cases of refractory epilepsy(TLE)patients were included into the study. All patients underwent PET before comparison with surgical findings and magnetoencephalography. At the same time,15 normal healthy volunteers were screened by history,general physical examination and routine labo-ratory testing. Results All of the patients with MRI were treated using same protocol with as a diagnosis of epilepsy without MRI. 12 patients were completely normal(1 patients in the first leaf showed very small abnormalities). The plasma free fraction(F1)of 18 F - FCWAY was higher than that in healthy controls(0. 13 ± 0. 04 vs. 0. 09 ± 0. 07,P ﹤ 0. 10). In patients with focal MEG stimulation(n = 8),all 8 cases had focal sei-zures. Unilateral MEG stimulation area was found in 7 cases,while 3 cases had focal seizure. Non unilateral MEG stimulation area in 2 cases had no focal seizures. Conclusion While the results of MRI - and PET/ MEG were consistent,and the results were more accurate in the preoperative evaluating for patients with negative - MRI interact epilepsy.
