目的 利用Bold-fMRI研究颞叶癫痫(TLE)患者与正常对照组在语言任务模式下脑激活的差异,探讨颞叶癫痫患者脑语言功能改变的影像学表现.方法 对经过手术病理证实的17例发作间期右利手TLE患者及相匹配14例正常对照者行动词生成语言任务的脑部Bold-fMRI检查.采用SPM 8软件对Bold-fMRI脑功能图像进行数据处理和分析,采用两样本t检验比较癫痫组和对照组动词生成语言任务的fMRI激活图差异.结果 Bold-fMRI脑激活结果显示,左侧颞叶癫痫组与对照组比较脑功能激活减低的区域有双侧额下回、颞下回、双侧枕叶、双侧小脑半球、双侧岛叶、前扣带回、双侧中央前回、辅助运动区及左侧海马、舌回,激活增高的区域为右侧楔前叶;右侧颞叶癫痫组与对照组比较,脑功能激活减低的区域有右侧额下回、中央后回及岛叶.说明左侧颞叶癫痫对语言相关功能区的影响较大.结论 颞叶癫痫患者动词生成任务Bold-fMRI脑功能激活范围及强度较健康对照组明显降低,以左侧颞叶癫痫者为著.

Objective To study the difference in the brain activation under the language task between patients with temporal lobe epilepsy(TLE)and the normal people using Blood oxygen level dependent-functional magnetic resonance imaging(BOLD-fMRI), and to investigate the language impairment of the temporal lobe epilepsy patients.Methods Seventeen right-handed patients with pathologic confirmed TLE and Fourteen right-handed healthy volunteers were recruited for the study. BOLD-fMRI were performed with verbal tasks. The BOLD-fMRI brain images were analyzed using SPM8, and two-samplet test was performed to compare the difference of language features and the brain activation between the epilepsy group and the control group. Results In verbal generation task, compared with the control group, negative activated differences were found in the group with left TLE in the following brain regions: bilateral inferior frontal gyrus, inferior temporal gyrus, bilateral occipital lobe, bilateral cerebellar hemispheres, bilateral anterior cingulate gyrus, insula, supplementary motor area and the left hippocampus, lingual gyrus. While in the group with right TLE, the negative activated differences were found in right inferior frontal gyrus, right postcentral gyrus and right insula. Conclusion The BOLD-fMRI brain activation range and intensity of the patients with left temporal lobe epilepsy were significantly lower than that of the healthy controls, especially in patients with left temporal lobe epilepsy.
