目的:探讨磁共振弥散加权成像及T2加权像对肝脏局部病灶病变诊断的准确性。方法将57例肝占位患者根据病理结果分为良性组和恶性病变组,应用磁共振弥散加权成像及T2加权像对病变部位的弥散系数进行计算,观察其诊断准确性。结果①肝囊肿、血管瘤和肝恶性肿瘤(包括肝细胞癌和转移性肿瘤)三者之间平均弥散系数差异具有统计学意义(P<0.05);而肝细胞癌和转移性肿瘤两者平均弥散系数差异无统计学意义(P>0.05)。表面弥散系数诊断恶性肿瘤的敏感性为85.7%(24/28),特异性为97.8%(44/45),诊断准确性为93.2%(68/73);②血管瘤和囊肿T2平均值明显高于恶性病变,差异有统计学意义(P<0.05)。T2值诊断的敏感性为80.0%(24/30),特异性为91.7%(44/48),诊断准确性为87.2%(68/78)。结论磁共振弥散加权成像利用平均弥散系数差异和T2加权像能够提高局灶病变诊断的准确性。
目的:探討磁共振瀰散加權成像及T2加權像對肝髒跼部病竈病變診斷的準確性。方法將57例肝佔位患者根據病理結果分為良性組和噁性病變組,應用磁共振瀰散加權成像及T2加權像對病變部位的瀰散繫數進行計算,觀察其診斷準確性。結果①肝囊腫、血管瘤和肝噁性腫瘤(包括肝細胞癌和轉移性腫瘤)三者之間平均瀰散繫數差異具有統計學意義(P<0.05);而肝細胞癌和轉移性腫瘤兩者平均瀰散繫數差異無統計學意義(P>0.05)。錶麵瀰散繫數診斷噁性腫瘤的敏感性為85.7%(24/28),特異性為97.8%(44/45),診斷準確性為93.2%(68/73);②血管瘤和囊腫T2平均值明顯高于噁性病變,差異有統計學意義(P<0.05)。T2值診斷的敏感性為80.0%(24/30),特異性為91.7%(44/48),診斷準確性為87.2%(68/78)。結論磁共振瀰散加權成像利用平均瀰散繫數差異和T2加權像能夠提高跼竈病變診斷的準確性。
목적:탐토자공진미산가권성상급T2가권상대간장국부병조병변진단적준학성。방법장57례간점위환자근거병리결과분위량성조화악성병변조,응용자공진미산가권성상급T2가권상대병변부위적미산계수진행계산,관찰기진단준학성。결과①간낭종、혈관류화간악성종류(포괄간세포암화전이성종류)삼자지간평균미산계수차이구유통계학의의(P<0.05);이간세포암화전이성종류량자평균미산계수차이무통계학의의(P>0.05)。표면미산계수진단악성종류적민감성위85.7%(24/28),특이성위97.8%(44/45),진단준학성위93.2%(68/73);②혈관류화낭종T2평균치명현고우악성병변,차이유통계학의의(P<0.05)。T2치진단적민감성위80.0%(24/30),특이성위91.7%(44/48),진단준학성위87.2%(68/78)。결론자공진미산가권성상이용평균미산계수차이화T2가권상능구제고국조병변진단적준학성。
Objective To explore the accuracy of diffusion weighted magnetic resonance imaging and T2 weighted imaging in the diagnosis of focal liver lesion. Methods Fifty-seven patients of liver lesions were divided into benign group and malignant group according to the pathological results. The diffusion coefficient of the lesion was calculated using diffusion-weighted magnetic resonance imaging and T2 weighted imaging. The diagnosis accura-cy of focal liver lesions were observed. Results The liver cyst, hemangioma and hepatic malignant tumors (includ-ing hepatocellular carcinoma and metastatic tumor) showed statistically significant difference in average diffusion co-efficient (P<0.05), while there was no significant difference in the average diffusion coefficient between hepatocellu-lar carcinoma and metastatic tumor (P>0.05). The sensitivity of the surface diffusion coefficient was 85.7% (24/28) for diagnosis of malignant tumor, and the specificity was 97.8%(44/45), with diagnostic accuracy of 93.2%(68/73). The T2 average value of hemangioma and liver cyst was significantly higher than that of malignant lesions (P<0.05). The diagnosis sensitivity of T2 value was 80.0% (24/30), and the specificity and diagnostic accuracy were 91.7%(44/48) and 87.2%(68/78). Conclusion Diffusion weighted magnetic resonance imaging using the average diffusion coefficient difference and T2 weighted imaging can improve the diagnostic accuracy of focal lesions.
