目的 系统评价呼出气一氧化氮在咳嗽变异型哮喘患者中检测的临床意义.方法 通过计算机检索Pubmed、EMBASE、Medline、Cochrane databases、中国知网、万方及维普数据库中关于咳嗽变异型哮喘患者检测呼出气一氧化氮的文献,进行文献质量评估并提取数据,运用RevMan 5.0进行meta分析.结果 纳入10篇文献,共1 158例患者.Meta分析结果显示:咳嗽变异型哮喘患者呼出气一氧化氮水平较健康对照者、嗜酸粒细胞性支气管炎患者、其他慢性咳嗽疾病患者明显升高,加权均数差(WMD)分别为40.20(95%CI12.11~68.30),21.87(95%CI 16.29~27.44),31.01(95%CI:23.82~38.19);与典型支气管哮喘比较是明显下降的,且WMD为-53.75(95%CI-69.95~-37.55).结论 呼出气一氧化氮检测是一种简单、无创的气道炎症检测方法.其在慢性咳嗽的鉴别诊断中,特别是咳嗽变异型哮喘的诊断中具有重要的临床意义.

Objective The purpose of this meta-analysis is to evaluate fractional exhaled nitric oxide (FeNO) in cough variant asthma (CVA).Methods A systematic search was performed in Pubmed,EMBASE,Medline, Cochrane databases, CNKI, Wanfang and Weipu to find relevant studies all studies that reported FeNO in CVA were included.A meta-analysis were performed using RevMan 5.0.Results One thousand one hundred and fifty-eight patients from 10 studies were included in the meta-analysis.The results of the meta-analysis showed that FeNO level was significantly higher in CVA patients than in controls (WMD =40.20,95% CI 12.11-68.30), eosinophilic bronchitis (WMD =21.87,95% CI 16.29-27.44) and other chronic cough patients(WMD=31.01,95% CI 23.82-38.19).However,FeNO level was significantly lower in CVA patients than in bronchial asthma patients(WMD=53.75,95% CI-69.95-37.55).Conclusions FeNO might be a potentially useful non-invasive biomarker of airway inflammation in CVA.FeNO is very important for differential diagnosis of chronic cough in clinic practices.
