目的：探讨钬激光碎石术（HLL）在老年泌尿系统结石中的应用。方法：选取118例老年泌尿系统结石患者，分为HLL组和PL组，分别采取HLL和气压弹道碎石术（PL）治疗，观察疗效。结果：HLL组一次性碎石率（91.38%）高于PL组（80.00%）；8周后，HLL组结石清除率（87.93%）高于PL组（71.67%）；两组术中出血量无统计学差异，HLL组手术时间、术后并发症均低于PL组（（41.25±2.31）minvs（62.37±4.22）min，P<0.05；8.62%vs 15.00%， P<0.05）。结论：HLL治疗老年泌尿系统结石，结石清除率高，疗效显著。

목적：탐토화격광쇄석술（HLL）재노년비뇨계통결석중적응용。방법：선취118례노년비뇨계통결석환자，분위HLL조화PL조，분별채취HLL화기압탄도쇄석술（PL）치료，관찰료효。결과：HLL조일차성쇄석솔（91.38%）고우PL조（80.00%）；8주후，HLL조결석청제솔（87.93%）고우PL조（71.67%）；량조술중출혈량무통계학차이，HLL조수술시간、술후병발증균저우PL조（（41.25±2.31）minvs（62.37±4.22）min，P<0.05；8.62%vs 15.00%， P<0.05）。결론：HLL치료노년비뇨계통결석，결석청제솔고，료효현저。
ObjectiveTo investigate the application of holmium laser lithotripsy (HLL) in urinary calculus in the old pa-tients.Methods 118 cases of elderly patients with urinary calculi were selected and divided into group HLL and group PL. They were respectively given the treatment of the holmium laser lithotripsy (HLL) and the pneumatic ballistic lithotripsy (PL). The curative effect was observed.Results The rate of disposable lithotripsy of the HLL group was the one 91.38%, which was higher than that (80%) of the PL group. Eight weeks after the treatment, the stone clearance rate of the HLL group was 87.93%, which was higher than that of the PL group. The amount of bleeding during the operation in the two group had no statistical dif-ference. The operation time (41.25 + 2.31) minutes and the incidence of complications (8.62%) of the HLL group during and after operation were less than those [(62.37±4.22) minutes and 15.00%] of the PL groupConclusion The stone clearance rate of the treatment of the holmium laser lithotripsy (HLL) in urinary calculus in the old patients is higher and its curative effect is remarkable.
