目的：总结经骶尾入路手术在低位直肠良性病变及直肠肿瘤局部切除中的应用体会。方法低位直肠疾病患者18例，均采用经骶尾入路手术治疗。直肠类癌4例，直肠间质瘤4例、直肠癌4例（Dukes 分期均为 A 期）、直肠腺瘤2例、直肠内脱垂合并盆底疝2例、直肠后囊肿1例、直肠阴道瘘1例。结果术后随访发现，所有患者均无瘤存活。2例直肠内脱垂合并盆底疝，术后疗效差，其中1例再次行全结肠切除术。结论骶尾入路手术简单易行，手术创伤小，病变切除完整，有利于保护性功能和肛门功能。
목적：총결경저미입로수술재저위직장량성병변급직장종류국부절제중적응용체회。방법저위직장질병환자18례，균채용경저미입로수술치료。직장유암4례，직장간질류4례、직장암4례（Dukes 분기균위 A 기）、직장선류2례、직장내탈수합병분저산2례、직장후낭종1례、직장음도루1례。결과술후수방발현，소유환자균무류존활。2례직장내탈수합병분저산，술후료효차，기중1례재차행전결장절제술。결론저미입로수술간단역행，수술창상소，병변절제완정，유리우보호성공능화항문공능。
Objective To discuss the value of trans-sacral(Kraske)approach for the local re-section of lower rectal lesions. Methods A total of eighteen patients with lower rectal diseases underwent trans-sacral approach and followed up for evaluation of curative effect. Results There were one rectal cyst,two rectal adenoma,two internal rectal prolapse combined with pelvic floor hernia,four rectal carci-noid,four rectal stromal tumor and four rectal cancer(Dukes A). During the follow-up period,all pa-tients survived without tumor. Patients with internal rectal prolapse combined with pelvic floor hernia had poor treatment response and one of them received additional total colectomy. Conclusions Trans-sacral approach can simplify the procedure,minimize trauma,completely remove lesions,and preserve sexual and anal function.
