目的:探讨不同胆红素浓度对血浆凝血酶原时间(PT)和活化部分凝血活酶时间(APTT)检测结果的影响.方法:对含有不同总胆红素浓度的质控血浆进行血浆凝血酶原时间和活化部分凝血活酶时间指标测定,比较不同浓度的胆红素对上述指标的影响.结果:随着血浆总胆红素浓度的升高,PT、APTT检测结果升高,胆红素水平低于20 μmol/L时,对血浆PT、APTT测定结果影响的差异无统计学意义(P>0.05),当胆红素浓度高于41 μmol/L时,APTT检测结果显著高于对照血浆水平(P<0.05),胆红素浓度在 62 μmol/L 以上时,PT 检测结果显著高于对照血浆水平(P<0.05).结论:血浆胆红素水平低于 20 μmol/L 时,对凝血功能检测结果影响不大,但血浆胆红素浓度显著升高时,会对PT、APTT检测结果产生干扰,当单纯因胆红素水平升高导致PT、APTT结果异常升高时,可用回归方程加以校正,以便为临床提供更加准确的检测结果.

Objective:To explore the influence of different bilirubin concentrations on the plasma prothrombin time(PT) and activated partial thromboplastin time(APTT) test results.Methods:The plasma prothrombin time and activated partial thromboplastin time indexes of the quality control serum containing different bilirubin concentrations were measured.The influences of different bilirubin concentrations on above indexes were compared.Results:With the increase of serum total bilirubin concentration,the APTT and PT test results were increased.When the bilirubin level was less than 20 μmol/L,the influence on plasma APTT and PT had no statistical significance(P>0.05).When the bilirubin concentration was more than 41 μmol/L,the APTT test result was significantly higher than the controlled plasma level(P<0.05).When the bilirubin concentration was more than 62 μ mol/L,the PT test result was significantly higher than the controlled plasma level(P<0.05).Conclusion:When the bilirubin level is less than 20 μ mol/L,the influence on the coagulation function test results is not big.However the plasma bilirubin concentration is significantly increased,it has distraction on the PT and APTT test results.When simply because of elevated bilirubin levels lead to PT, APTT results abnormal increase,it could be used the regression equation for correction to provide more accurate detection results in clinic.
