目的:本文旨在研究胆固醇酯转运蛋白(cholesteryl ester transfer protein,CETP)D442G基因多态性与脑梗死发病的相关性. 方法:选取包头医学院第一附属医院神经内科脑梗死患者180例为观察组,体检科体检人员130例为对照组. DNA试剂盒提取外周血DNA,采用聚合酶链反应-限制性片段长度多态性( polymerase chain reaction-restriction fragment length poly-morph,PCR-RFLP)方法检测CETP D442G的基因型,分析D442G基因型与脑梗死发病的关系. 结果:D442G基因位点DG基因型高密度脂蛋白水平高于GG、DD基因型,差异有统计学意义( P <0. 05). 以是否发生脑梗死作为应变量,以年龄、性别、体重指数、血脂水平、糖尿病、高血压、心脏病、CETP D442G基因型及等位基因频率作为自变量,应用非条件logistic回归分析得出年龄、高血压、糖尿病、甘油三酯、胆固醇、低密度脂蛋白为脑梗死发病的独立危险因素( OR >1, P <0. 05);高密度脂蛋白、DG基因型为脑梗死发病的保护因素( OR <1, P <0. 05). 结论:脑梗死发病的危险因素可能为年龄、吸烟史、高血压病史、糖尿病病史、胆固醇、低密度脂蛋白、甘油三酯水平,脑梗死发病的保护因素可能为D442G基因DG基因型及高密度脂蛋白水平.

Objective:To study the relationship between cholesteryl ester transfer protein D442G polymorphism and cerebral infarc-tion. Methods:180 patients with cerebral infarction hospitalized in Neurology Department of the First Affiliated Hospital of Baotou Medical College were selected as the observation group and 130 healthy personnel in the Physical Examination Department as the control group. DNA was extracted from peripheral blood and the genotype of CETP D442G was detected by polymerase chain reaction-restriction frag-ment length polymorph (PCR-RFLP) method, the relationship between the genotype of D442G and cerebral infarction was analyzed. Re-sults:The high-density lipoprotein level of the DG genotype at the D442G locus was significantly higher than that of GG and DD geno-type, the difference being significant ( P <0. 05). Whether cerebral infarction would occur was used as a dependent variable and age, gender, body mass index, blood lipid levels, diabetes, hypertension, heart disease, the genotype of CETP D442G and allele frequency were used as variables;non-conditional logistic regression analysis was used to get the results that age, hypertension, diabetes, triglycer-ide, cholesterol and low density lipoprotein were the independent risk factors for the incidence of cerebral infarction( OR >1, P <0. 05) while high density lipoprotein and the DG genotype were the protective factors for the incidence of cerebral infarction ( OR <1, P <0. 05 ) . Conclusion:The independent risk factors for the incidence of cerebral infarction age, history of smoking, hypertension and diabetes, triglyceride levels, cholesterol and low density lipoprotein and the protective factors for the incidence of cerebral infarction may be high density lipoprotein and the DG genotype of D442G.
