Atracurium%neuromuscular blockade%temperature%time factors
目的:评价不同贮存方法对苯磺顺阿曲库铵肌松效应的影响。方法择期全麻手术患者90例,采用随机数字表法将患者分为3组(n=30):低温组(A组)、常温组(B组)、常低温交替组(C组)。苯磺顺阿曲库铵置于低温冰箱2~4℃贮存30d(A组)、常温(22~25℃)存放30d(B组)或每天12h常低温交替存放30d(C组)。采用TOF-Watch SX肌松监测仪,采用单次超强电刺激观察拇内收肌的肌松程度。意识消失后静脉注射苯磺顺阿曲库铵0.2 mg/kg,当肌颤搐达到最大抑制时行气管插管。记录苯磺顺阿曲库铵起效时间、临床作用时间、恢复指数和75%恢复时间并评价气管插管条件。结果与A组比较,B组、C组苯磺顺阿曲库铵起效时间延长,临床作用时间、75%恢复时间和恢复指数缩短( P<0.05)。 A组气管插管条件优良,B组、C组气管插管条件差,差异有统计学意义( P<0.05)。结论室温贮存及常低温交替贮存30 d均可降低苯磺顺阿曲库铵肌松效应。

Objective To evaluate the efects of diferent storage methods on cisatracurium-in-duced neuromuscular block.Methods Ninety ASA I orⅡpatients,aged 20~53,scheduled for e-lective gynecological operations under general anesthesia,were randomly divided into 3 groups ( n=30 each):cisatracurium stored for 30 days at 2~4℃group( group A) ,cisatracurium stored for 30 days at room temperature group ( group B) and cisatracurium stored for 30 days at room temperature group and 2~4℃,12 h in turn ( group C) .Anesthesia was induced with iv injection of midazolam and target-controlled infusion of propofol( target plasma concentration 3μg/mL) and remifentanil( target efect-site concentration 3~5 ng/mL) .A bolus of cisatracurium 0.2 mg/kg was given intravenously over 5~10 S as soon as the patients lost consciousness.Neuromus-cular block was monitored with TOF-Watch SX( Organon,Netherlands) .Single stimulation was applied to the ulnar nerve at wrist.The maximal degree of neuromuscular block,onset time,clin-ical duration,recovery index and 75%recovery time were recorded.The patients were intubated and mechanically ventilated when neuromuscular block reached the maximal degree.The intuba-tion condition was evaluated.Results Compared with group A,the onset time was significantly prolonged,clinical duration, recovery index and 75%recovery time were shortened in group B and group C ( P<0.05) .The intubation condition was excellent or good in group A and there was no significant difference among group B and group C.Conclusion The neuromuscular block of ci-satracurium is significantly attenuated when stored for 30 days at room temperature or room temperature and 2~4℃alternately.
