目的:探讨以消化系统为中心的整合科室的护理管理方法。方法将北京大学第一医院消化内科、胃肠外科、内镜中心整合为消化系统科室,对消化系统疾病患者实施内外科整合,内镜、微创、手术结合的一体化医疗和护理。对整合后科室进行护理整合管理,动态弹性调配各整合病房的护士,内外科护士交叉培训、交叉疾病护理流程标准化、健康教育集体化、教学科研共享化等。结果以消化系统为中心的科室整合后,护士专科操作成绩从(84.75±3.26)分提高至(95.35±1.54)分,差异有统计学意义(t=13.634,P<0.01);患者满意度从94.3%提高至97.2%,差异有统计学意义(χ2=5.918,P<0.05)。结论以消化系统为中心整合科室的护理管理体现了以患者为中心的护理模式,促进了多学科护理团队的发展,内外科护士优势互补,相互融合,在提高护士专科护理能力、提高患者满意度方面具有重要意义。

목적:탐토이소화계통위중심적정합과실적호리관리방법。방법장북경대학제일의원소화내과、위장외과、내경중심정합위소화계통과실,대소화계통질병환자실시내외과정합,내경、미창、수술결합적일체화의료화호리。대정합후과실진행호리정합관리,동태탄성조배각정합병방적호사,내외과호사교차배훈、교차질병호리류정표준화、건강교육집체화、교학과연공향화등。결과이소화계통위중심적과실정합후,호사전과조작성적종(84.75±3.26)분제고지(95.35±1.54)분,차이유통계학의의(t=13.634,P<0.01);환자만의도종94.3%제고지97.2%,차이유통계학의의(χ2=5.918,P<0.05)。결론이소화계통위중심정합과실적호리관리체현료이환자위중심적호리모식,촉진료다학과호리단대적발전,내외과호사우세호보,상호융합,재제고호사전과호리능력、제고환자만의도방면구유중요의의。
Objective To investigate the nursing integrated management practice methods for the digestive system as center of integration department. Methods The digestive medicine, gastrointestinal surgery and digestive endoscopy center were integrated into the digestive system integration department, which the patients of digestive diseases could accept treatment, nursing integrated medicine and surgery including endoscopy, minimally invasive treatment and operation. The nursing integrated management was implemented to the integration department, such as deploying nurses in integrated wards elastically, cross-training nurses between digestive medicine and gastrointestinal surgery, implementing cross-disease care process standardization and health education collectivization, sharing teaching and research resources. Results After the integration, the score of nurses′specialist operation improved from (84. 75 ± 3. 26) to (95. 35 ± 1. 54) (t=13. 634,P<0. 01);patients′satisfaction improved from 94. 3% to 97. 2% (χ2 =5. 918,P<0. 05). Conclusions Nursing integrated management for the digestive system centered integration department demonstrates client-centered nursing model which can promote the development of multidisciplinary nursing team, achieve complementary advantages and fusion between medical nurses and surgical nurses. It is of great significance in improving the specialized care ability of nurses and patients′satisfaction.
