目的探讨瑞芬太尼联合丙泊酚麻醉对腹腔镜胆囊切除术患者镇痛、血流动力学及不良反应的影响.方法将2014年10月至2015年5月医院收治的腹腔镜胆囊切除术患者80例,按随机数字法分为对照组(芬太尼联合丙泊酚麻醉)和观察组(瑞芬太尼联合丙泊酚麻醉),各40例.结果与对照组相比,观察组拔管后5 min,10 min的清醒/镇静状态分级评分标准(OAA/S评分)均明显增高( P<0. 05);与对照组相比,观察组气腹后、术后心率(HR)、收缩压(SBP)、舒张压(DBP)均有明显降低( P<0. 05);与对照组相比,观察组麻醉期间不良反应发生率显著降低( P<0. 05).结论瑞芬太尼联合丙泊酚麻醉能稳定血流动力学变化,发挥较好的麻醉效果,减少药品不良反应.

Objective To study the effect of remifentani combined with propofol on anesthesia analgesia, hemodynamic and adverse re-actions after laparoscopic cholecystectomy. Methods 80 patients with laparoscopic cholecystectomy in the hospital during the period from Oct 2014 to May 2015 were divided into the control group ( fentani combined with propofol ) and the observation group ( remifentani combined with propofol ) according to the random number method, 40 cases in group. Results Compared with the control group, the OAA/S scores at 5 min and 10 min after extubation were significantly increased in the observation group ( P < 0. 05 );the HR, SBP, DBP levels on post-pneumoperitoneum and post-operation were significantly decreased in the observation group ( P < 0. 05 );the incidence of adverse reactions during the anesthesia period was obviously reduced in the observation group ( P < 0. 05 ) . Conclusion Remifentani combined with propofol can stabilize the hemodynamic changes for patients with laparoscopic cholecystectomy, has a better anesthetic effect, and can reduce the adverse reactions during the anesthesia period.
