目的 探讨在扁桃体切除术全身麻醉中,采用不同剂量地佐辛对患者苏醒期的影响.方法 选取2013年6月至2015年1月确诊并接受扁桃体切除术的患者 120 例,按随机数字表法将患者进行分组,A 组患者给予 0. 05 mg/kg 的地佐辛滴注,B组患者给予0. 1 mg/kg的地佐辛滴注,C组患者给予0. 2 mg/kg的地佐辛滴注.结果 A组、B组在恢复呼吸时间、苏醒时间及拔出气管导管时间上无显著性差异( P>0. 05),与C组相比具有显著性差异( P<0. 05);C组患者的躁动评分(1. 64 ± 0. 53)分和疼痛评分(1. 85 ± 0. 63)分,显著低于A组的(2. 65 ± 0. 84)分和(5. 21 ± 1. 73)分( P <0. 05);3 组恶心呕吐、心动过缓、呼吸困难及睡眠过度发生率等临床不良反应相比,无显著性差异( P >0. 05).结论 地佐辛在扁桃体切除术中具有一定的抗躁动和抗疼痛作用,但不同剂量的作用效果具有一定差异,0. 1 mg/kg的滴注量更安全、有效.

Objective To investigate the effect of different doses of dezocine on agitation during analepsia period of tonsillectomy with general anesthesia. Methods 120 cases of patients from June 2013 to January 2015 in the hospital who performed tonsillectomy were divided according to the random number table method. The group A was given dezocine drops 0. 05 mg/kg, the group B 0. 1 mg/kg, the group C of 0. 2 mg/kg. Results The recovery of respiratory time, awakening time and extubation time in group A and B had no significant difference, and had statistical difference compared with group C ( P < 0. 05 );the agitation score of the group C was ( 1. 64 ± 0. 53 ) and the pain score was ( 1. 85 ± 0. 63 ) , which were significantly lower than ( 2. 65 ± 0. 84 ) and ( 5. 21 ± 1. 73 ) of group A;the adverse reaction of nausea and retching, heartbeat bradycardia, difficulty breathing and sleep over of the three groups had no significant difference ( P < 0. 05 ) . Conclusion Dezocine has certain anti agitation and anti pain effect in tonsillectomy, but the effects and adverse reactions of different dose have some differences;the application amount of 0. 1mg/kg in the clinical infusion during general anesthesia is more safe and effective.
