目的 了解医院2012年至2013年卡马西平血药浓度监测情况,分析卡马西平血药浓度影响因素,为卡马西平临床合理用药提供依据.方法 采用高效液相色谱( HPLC )法对癫痫患者进行卡马西平血药浓度监测,并对监测结果进行统计和分析.结果 210例次卡马西平血药浓度监测中,低于4μg/mL 57例次( 27. 14%) ,4~12μg/mL 152例/次( 72. 38%) ,超过12μg/mL仅1例次( 0. 48%);卡马西平血药浓度与年龄、联合用药存在明显的相关性( P<0. 05 );性别对血药浓度的影响不明显( P>0. 05 ).结论 开展卡马西平血药浓度监测,对实施个体化给药方案,提高临床治疗水平和促进合理用药具有重要意义.

Objective To understand the monitoring result of serum concentration monitoring of carbamazepine in epileptic patients and analyze of influential factors with clinical efficacy of carbamazepine, to provide the reference for clinical rational drug use. Methods Serum concentrations of carbamazepine were determined by HPLC. The results were observed and analyzed. Results Of all the 210 cases monitored, blood concentration monitoring of carbamazepine less than 4 μg/mL were 57 cases, accounted for 27. 14%, 4-12 μg/mL were 152 cases, accounted for 72. 38%. Only 1 case ( 0. 48% ) of blood concentration was higher than 12 μg/mL. There was visible dependability between blood concentration of carbamazepine and ages and drug combination ( P < 0. 05 ) , blood concentration of carba-mazepine in men and women were similar. Conclusion Blood concentration monitoring of carbamazepine for long term usage patients, combined with clinical efficacy of carbamazepine and developed individualized regimens can boost the clinical iatreusis level and promote rational use of drug.
