目的 本研究旨在观察原发性三叉神经痛患者, 在CT引导定位半月神经节不同温度热凝毁损治疗与脉冲射频治疗下的效果对比及并发症的差异,并对疗效及安全性进行评估.方法 45例病人,均经过临床及CT或MRI检查确诊,随机分成A、B、C三个治疗组,每组15人.A组80℃,B组55℃,C组42℃,三组均操作120S,并重复二次.分别记录治疗前和治疗后1天、3天、1个月、3个月和6个月的VAS评分,疗效评价及不良反应的评估与比较.结果 80℃热凝毁损疼痛缓解最优,相比差异有统计学意义(P<0.05),但并发症较多;55℃热凝毁损与42℃脉冲治疗效果差异相近,相比差异无统计学意义(P>0.05);但42℃脉冲治疗后并发症最少.结论 80℃热凝毁损治疗成功率高、效果确切,但也发生诸多并发症;42℃脉冲治疗可有效的缓解患者疼痛,术后并发症少,安全性高,有待进一步探讨.

목적 본연구지재관찰원발성삼차신경통환자, 재CT인도정위반월신경절불동온도열응훼손치료여맥충사빈치료하적효과대비급병발증적차이,병대료효급안전성진행평고.방법 45례병인,균경과림상급CT혹MRI검사학진,수궤분성A、B、C삼개치료조,매조15인.A조80℃,B조55℃,C조42℃,삼조균조작120S,병중복이차.분별기록치료전화치료후1천、3천、1개월、3개월화6개월적VAS평분,료효평개급불량반응적평고여비교.결과 80℃열응훼손동통완해최우,상비차이유통계학의의(P<0.05),단병발증교다;55℃열응훼손여42℃맥충치료효과차이상근,상비차이무통계학의의(P>0.05);단42℃맥충치료후병발증최소.결론 80℃열응훼손치료성공솔고、효과학절,단야발생제다병발증;42℃맥충치료가유효적완해환자동통,술후병발증소,안전성고,유대진일보탐토.
Objective To observe and compare the effect and complication of different temperatures radio frequency with pulsed radio frequency on ganglion under CT guided in the patients with primary trigeminal neuralgia, and then evaluate the efficacy and safety. Methods 45 patients whom were clearly diagnosed with trigeminal neuralgia by CT or MRI were randomly divided into A, B, C groups. 15 patients in each group. Group A 80℃, Group B 55℃, Group C 42℃, Treatment lasted 120s in all three groups, and repeated twice. VAS scores and appraisal of efficacy and complication were collected 1 day before treatment, 1 day, 3 days, 1 month, 3 months, and 6 months after treatment. Results 80℃group received the optimal pain relief effect. There was statistically significant (P<0.05), but also had more complications. The effect was similar in 55℃and 42℃groups, and the difference was not statistically significant (P>0.05), but in 42℃group the complication was least. Conclusion 80℃radio frequency had high success rate and exact effect, but also with more complications;42℃ pulse treatment can effectively relieve pain,high safety,and with less postoperative complications, need to be further explored.
