目的 观察闭合复位与股骨近端防旋髓内钉(PFNA)内固定用于老年股骨粗隆间骨折治疗的效果.方法 选择62例股骨粗隆间骨折老年患者,分为治疗组和对照组.治疗组32例行闭合复位与PFNA治疗,对照组30例行切开复位与动力髋螺钉(DHS)治疗,比较两组各项临床指标,观察PFNA与闭合复位的疗效.结果 治疗组手术时间、术中出血量、术后活动时间、Harris评分分别为(41.25±3.50)min、(47.86±3.30)ml、(3.18±1.15)d、(87.95±3.58)分,对照组分别为(93.52±4.65)min、(490.15±8.02)ml、(18.60±2.38)d、(87.94±3.29)分,治疗组前三项指标均低于对照组,差异有统计学意义(P ＜0.05),最后一项指标两组比较差异未见统计学意义(P＞0.05).结论 股骨粗隆间骨折的老年患者行闭合复位、PFNA内固定治疗,可以有效地缩短手术时间,减少术中出血量,加快术后恢复进程.

Objective To observe the effect of closed reduction and proximal femoral nail anti-rotation(PFNA) on elderly patients with intertrochanteric fracture.Methods Sixty elderly patients with intertrochanteric fractures were divided into treatment group and control group.The 32 cases in treatment group were treated by closed reduction and PFNA, and 30 cases in control group were treated by open reduction and dynamic hip screw(DHS).The clinical parameters were compared, and the effects of PFNA with closed reduction were observed.Results Treatment operative time, blood loss, postoperative activity time, Harris scores were (41.25 ±3.50)min, (47.86 ±3.30)ml, (3.18±1.15) d, (87.95 ±3.58) points in the treatment group, and (93.52 ±4.65) min, (490.15 ±8.02)ml, (18.60 ±2.38)d, (87.94 ± 3.29) points in the control group.The operative time, blood loss, postoperative activity time in treatment group were significantly lower than those in control group (P ＜0.05), but the Harris scores had no significant difference between the two groups (P ＞ O.05).Conclusions Fracture closed reduction and PFNA fixation for treating elderly patients with intertrochanteric fracture can effectively shorten the operation time, reduce blood loss, and accelerate the process postoperative recovery.
