目的 研究瑞芬太尼复合丙泊酚对腹腔镜胆囊切除术(LC)患者血流动力学的影响.方法 将驻马店市中心医院94例行LC手术患者随机分为观察组与对照组,观察组采取瑞芬太尼复合丙泊酚治疗,对照组静吸复合麻醉,比较两组麻醉各时间段的血流动力学指标改变及恢复时间.结果 观察组睁眼时间、拔管时间、自主呼吸时间、定向力恢复时间、离开恢复室时间分别为(5.21±1.52)、(7.20±2.71)、(3.58±1.42)、(7.21±2.84)、(17.61±3.51) min,对照组分别为(8.80±4.31)、(11.58±3.80)、(8.24±2.64)、(11.60±3.20)、(29.61±4.10) min,两组比较差异均有统计学意义(P＜0.05);观察组T1～T4各时间段收缩压、心率均显著低于对照组(P＜0.05),舒张压、血氧饱和度与对照组比较差异未见统计学意义(P＞0.05).结论 瑞芬大尼复合丙泊酚麻醉在腹腔镜胆囊切除术中麻醉效果可靠,术后恢复较快,具有较高的临床应用价值.

목적 연구서분태니복합병박분대복강경담낭절제술(LC)환자혈류동역학적영향.방법 장주마점시중심의원94례행LC수술환자수궤분위관찰조여대조조,관찰조채취서분태니복합병박분치료,대조조정흡복합마취,비교량조마취각시간단적혈류동역학지표개변급회복시간.결과 관찰조정안시간、발관시간、자주호흡시간、정향력회복시간、리개회복실시간분별위(5.21±1.52)、(7.20±2.71)、(3.58±1.42)、(7.21±2.84)、(17.61±3.51) min,대조조분별위(8.80±4.31)、(11.58±3.80)、(8.24±2.64)、(11.60±3.20)、(29.61±4.10) min,량조비교차이균유통계학의의(P＜0.05);관찰조T1～T4각시간단수축압、심솔균현저저우대조조(P＜0.05),서장압、혈양포화도여대조조비교차이미견통계학의의(P＞0.05).결론 서분대니복합병박분마취재복강경담낭절제술중마취효과가고,술후회복교쾌,구유교고적림상응용개치.
Objective To study the effects of remifentanil combined with propofol on the hemodynamics of laparoscopic cholecystectomy during anesthesia.Methods Ninety-four patients implemented of laparoscopic cholecystectomy in Zhumadian central hospital were randomly divided into two groups.The control group were treated by general anesthesia and the test group were treated by remifentanil combined with propofol.The changes of hemodynamics and recovery time at different time during anesthesia were compared.Results The time points of test group when eye open, extubation, spontaneous breathing recovery, directional power and out of PACU were respectively (5.21 ± 1.52), (7.20 ± 2.71),(3.58 ± 1.42), (7.21 ± 2.84), (17.61 ± 3.51) min, while that in control group were respectively (8.80 ± 4.31), (11.58 ± 3.80), (8.24 ± 2.64), (11.60 ± 3.20), (29.61 ± 4.10), the differences were significant between the two groups (P ＜ 0.05).The systolic blood pressure and heart rate of test group were significantly lower than that in control group (P ＜ 0.05).There was no significant difference in diastolic blood pressure or blood oxygen saturation between the two groups (P ＞ 0.05).Conclusions Remifentanil combined with propofol anesthesia has reliable anesthetic effect on laparoscopic cholecystectomy, and is worth to be applied in clinics.
