目的探讨环磷酰胺长期治疗结缔组织病并间质性肺病(CTD-ILD)的效果.方法选择2007年1月至2013年12月曾经在唐山康复医疗中心风湿免疫科住院并确诊为CTD-ILD患者113例,应用随机数字表分为两组,环磷酰胺治疗组59例,雷公藤多苷治疗组54例.环磷酰胺治疗组应用环磷酰胺50～ 100 mg/d.雷公藤多苷治疗组应用雷公藤多苷片40～ 60 mg/d.观察期2年.对上述患者治疗前和观察终点分别通过肺高分辨率CT(HRCT)进行肺部病变范围定量比较.结果治疗前环磷酰胺治疗组HRCT评分(2.53±1.06)分,雷公藤多苷治疗组评分(2.70±1.36),两组比较差异无统计学意义(t=0.73,P＞0.05);治疗后环磷酰胺治疗组评分(2.00±1.03)分,雷公藤多苷治疗组评分(2.43±1.13)分,两组比较差异有统计学意义(t=2.51,P＜0.05).结论环磷酰胺治疗CTD-ILD效果明显,可以作为长期治疗CTD-ILD的方案.
목적 탐토배린선알장기치료결체조직병병간질성폐병(CTD-ILD)적효과.방법 선택2007년1월지2013년12월증경재당산강복의료중심풍습면역과주원병학진위CTD-ILD환자113례,응용수궤수자표분위량조,배린선알치료조59례,뢰공등다감치료조54례.배린선알치료조응용배린선알50～ 100 mg/d.뢰공등다감치료조응용뢰공등다감편40～ 60 mg/d.관찰기2년.대상술환자치료전화관찰종점분별통과폐고분변솔CT(HRCT)진행폐부병변범위정량비교.결과 치료전배린선알치료조HRCT평분(2.53±1.06)분,뢰공등다감치료조평분(2.70±1.36),량조비교차이무통계학의의(t=0.73,P＞0.05);치료후배린선알치료조평분(2.00±1.03)분,뢰공등다감치료조평분(2.43±1.13)분,량조비교차이유통계학의의(t=2.51,P＜0.05).결론 배린선알치료CTD-ILD효과명현,가이작위장기치료CTD-ILD적방안.
Objective To investigate the effect of cyclophosphamide in the treatment of connective tissue disease (CTD-ILD).Methods One hundred and thirteen patients diagnosised as CTD-ILD who used to be hospitalized in Department of Rheumatism and Immunology of the Rehabilitation Medical Center of Thangshan from January 2007 to December 2013.Fifty-nine cases among them of therapy of 50-100 mg/d cyclophosphamide(CYC) and 54 cases of 40-60 mg/d tripterygium wilfordii glycosides tablets, both the observation period was 2 year.A comparasion was made on the ration of lesions range showed on pulmonary Pulmonary high CT (HRCT) before and after treatments.Results Before treatment, there was no significant difference between CYC group and tripterygium wiffordii glycosides tablets group (2.53± 1.06 vs.2.70± 1.36, t=0.73,P＞ 0.05).Image of CYC group showed significant improvement, better than tripterygium wilfordii glycosides tablets group,and the difference between pulmonary HRCT reach statistical significance(2.00± 1.03 vs.2.43± 1.13, t =2.51, P＜0.05).Conclusion It is effective to treat CTD-ILD with CYC, and it can be used as a long-term solution for the treatment of CTD-ILD.
