目的 观察近日节律对靶控输注丙泊酚意识消失时半数有效浓度(EC50)的影响.方法 选择全身麻醉下手术患者60例,均分为日间组(D组,10:01~20:00)与夜间组(N组,20:01~10:00)(n=30),按序贯法调节丙泊酚靶浓度,初始血浆靶浓度为2.0μg/mL,每级靶浓度之差为0.5μg/mL,若患者意识消失,则下1患者采用低一级靶浓度,反之,则采用高一级靶浓度.结果 靶控输注丙泊酚时,日间与夜间EC50值及其95%可信区间分别为2.96(2.85~3.01)μg/mL和2.13(2.02~2.25)μg/mL,夜间EC50值低于日间组(P<0.05).结论 近日节律对靶控输注丙泊酚EC50值有影响,夜间EC 50值低于日间EC 50值.

목적 관찰근일절률대파공수주병박분의식소실시반수유효농도(EC50)적영향.방법 선택전신마취하수술환자60례,균분위일간조(D조,10:01~20:00)여야간조(N조,20:01~10:00)(n=30),안서관법조절병박분파농도,초시혈장파농도위2.0μg/mL,매급파농도지차위0.5μg/mL,약환자의식소실,칙하1환자채용저일급파농도,반지,칙채용고일급파농도.결과 파공수주병박분시,일간여야간EC50치급기95%가신구간분별위2.96(2.85~3.01)μg/mL화2.13(2.02~2.25)μg/mL,야간EC50치저우일간조(P<0.05).결론 근일절률대파공수주병박분EC50치유영향,야간EC 50치저우일간EC 50치.
Objective To investigate the effect of circadian rhythm on the iffty percent of effective concentration(EC 50) for propofol by TCI. Methods 60 patients scheduled for surgeries under general anesthesia were divided into 2 groups(n=30 each): day-time group (D group,from 10:01 to 20:00)and night-time group(N group, from 20:01 to 10:00). The initial target plasma concentration of propofol was set at 2.0μg /mL. The target concentration was adjusted by sequential method, the difference between each level was 0.5μg / mL. If the patient lost consciousness, then the next patient with a low-level target concentration ; on the contrary, with a high-level target concentration.Results The EC50 of propofol by TCI was 2.13μg /mL (95%CI 2.02-2.25μg /mL) in night group and 2.96μg /mL(95%CI 2.85-3.01μg /mL) in day group, the EC50 was signiifcantly lower in night group compared with day group.Conclusion Circadian rhythm has inlfuence on EC50 of propofol by TCI, EC50 value was lower in night than during the day time.
