目的:评估超声造影对坏疽性胆囊炎术前诊断的临床价值.方法:回顾性分析经手术病理证实的42例急性坏疽性和化脓性胆囊炎病例,术前行常规超声及造影检查,经两名资深超声医师完成图像特征分析及统计学分析.结果:急性坏疽性胆囊炎11例,单纯化脓性胆囊炎31例,并且11例中大部分是老年人.胆囊壁造影剂回声强度减弱,胆囊壁回声连续性中断及胆囊窝积液等征象在坏疽性胆囊炎与其他急性胆囊炎组间存在显著差异(p<0.05).明显缩短了患者检查到手术的间隔时间.结论:超声造影诊断坏疽性胆囊炎的声像图特征:胆囊壁内低灌注状态及囊壁的连续性中断,为临床早期诊断并及时手术治疗提供了重要信息.

Objective: To evaluate ultrasound CEUS for the preoperative diagnosis of gangrenous acute cholecystitis. Methods: Forty-two patients who underwent both US and CEUS and were confirmed as presenting with acute cholecystitis at pathology were included. Baseline US images and CEUS cine clips were analyzed by two experienced ultrasonograpy doctors. Statistical analyses were performed. Results: Gangrenous acute cholecystitis were diagnosis in 11 patients and uncomplicated acute cholecystitis in 31. Patients with gangrenous acute cholecystitis were found to be older.Three features including hypoenhanced gallbladder wall, discrete wall and pericholecystic fluid were found valuable in distinguish gangrenous cholecystitis form purulent cholecystitis with Statistical differences (p<0.05). Conclusions: CEUS is of great help in the diagnosis of gangrenous cholecystitis through some tipical features such as hypoenhanced and discrete of gallbladder wall.
