目的探讨开放式和闭合式两种阴道残端缝合方式对行经腹全子宫切除术患者术后康复的影响.方法将428例行经腹全子宫切除术的患者分为观察组(201例)与对照组(227例).观察组行开放式阴道残端缝合,对照组行传统闭合式阴道残端缝合,比较两组患者术后恢复情况.结果两组患者的术后发热时间、阴道残端出血、薇乔线吸收、阴道残端息肉的发生率差异有统计学意义(P<0.05).两组患者的手术时间、术中出血量、住院时间、阴道残端感染发生率、阴道残端愈合不良发生率、阴道长度及性生活满意率差异无统计学意义(P>0.05).结论开放式阴道残端缝合优于传统闭合式阴道残端缝合,利于患者术后康复.

Objective To discuss the effect of open saturation of vaginal stump and that of closed saturation in total abdominal hys-terectomy on the recovery of the patients.Methods 428 cases for total abdominal hysterectomy were divided into 2 groups:observation group (n=201) and control group (n=227);open saturation of vaginal stump was made to patients in observation group while close satu-ration of vaginal stump was made to patients in control group;a comparative study was made to the recovery of the patients between the 2 groups.Results There existed a difference of statistical significance in postoperative fever lasting time,bleeding of vaginal stump,ab-sorption of Vicryl suture and the occurrence of vaginal stump polyps between the 2 groups (P<0.05) while no difference in operation du-ration,bleeding volume in operation,time length of hospitalization,the occurrence of vaginal stump infection,the occurrence of poor healing of vaginal stump,vaginal length and the sexual life satisfaction rate between the 2 groups.Conclusions To make open sutura-tion of vaginal stump in total abdominal hysterectomy is superior to close suturation,and it is conducive to the recovery of the patients.
