目的探讨宫内节育器嵌顿/异位时各种腔镜的联合应用. 方法通过对30例患者宫内节育器嵌顿/异位诊治过程的回顾分析来观察联合腔镜技术的应用效果. 结果经宫腔镜检查后定位节育器直接取出的有15例;9例出现节育环嵌顿,通过宫腔镜检查后电切取出;5例异位于腹腔的患者,其中3例经腹盆腔分粘后取出,另2例因损伤直肠,术中取出节育环并行腹腔镜修补直肠;1例因考虑损伤膀胱术中联合膀胱镜检查后由宫腔镜取出. 结论当节育环取出困难,怀疑有节育器嵌顿/异位时,采用宫腹腔镜联合检查或宫膀胱镜联合诊治具有明显优势,在治疗过程中联合应用诊断更明确且更安全,值得临床推广.

Objective To explore the application of combined endoscopic techniques in the intrauterine device embed-ded or ectopic. Methods Collected 30 cases with intrauterine device embedded or ectopic , retrospectively analysed the pro-cess of diagnosis and treatment. Results 15 cases with intrauterine device were directly removed after hysteroscopy examina-tion location; 9 cases were removed by electricity cut after hysteroscopic; 5 cases located in the abdominal cavity, 3 cases of them after points stick out, 2 cases of damage to the rectum, then repair the rectum by laparoscopic in the operation; 1 case was taken out by hysteroscopy joint cystoscopy. Conclusion Used hysteroscopic and laparoscopic joint inspection or hys-teroscopy and cystoscope joint inspection, there are more clearly diagnosis and safer in the diagnosis and treatment of the in-trauterine device embedded or ectopic. It is worthy clinical promotion.
