目的:探讨异丙酚复合小剂量芬太尼在小儿肠镜检查中应用的效果及安全性.方法:将行肠镜检查的60例患儿随机分为2组,每组30例:A组(单用异丙酚);B组(异丙酚+芬太尼),观察两组各时段的麻醉效果(起效时间、苏醒时间)及术前、术中、术后血压(SBP)、心率(HR)、呼吸(R)、血氧饱和度(SpO2)变化.结果:A组患儿术中SBP、HR、R、SpO2与术前、术后相比均明显降低(P<0.05),差异有显著性;B组患儿术中SBP、HR、R、SpO2与术前、术后相比均有所降低,但仍在正常范围内(P>0.05),差异无显著性;两组间术中SBP、HR、R、SpO2比较差异有显著性(P<0.05);A组麻醉起效时间、苏醒时间与B组相比明显延长,差异有显著性(P<0.05).结论:异丙酚复合小剂量芬太尼不仅可以减少异丙酚用量、有效降低循环、呼吸抑制等不良反应,而且能缩短苏醒时间,使麻醉更平稳.

Objective:To investigate the efficiency and safety of propefol combined with small-dose fentanyl in children under enteroacopy. Methods:Sixty children (three to twelve years old) with enterescopy were randomly divided into two groups:group A (propofol)and group B (propofol combined with fentanyl),each group thirty cases. The anesthetic effect (onset time, recovery time) and blood pressure (SBP),heart rate (HR), respiration (R), blood oxygen saturation (SpO2) changes were observed during pre-operation, intra-operatiun and post-operation in 2 groups.Results:The intraoperative SBP, HR, R, SpO2 were significantly lower compared to pre-operation and post-operation level in group A (P<0.05), there was statistically significant difference. The intraoperative SBP,HR,R,SpO2 were lower compared to pre-operatiun and post-operation level in group B,but they were in the normal range (P>0.05),the differences were not statistically significant. The intraoperative SBP,HR,R,SpO2 differences were statistically significant in group A and group B(P< 0.05). The anesthesia onset time, recovery time in group A compared with the group B was significantly prolonged (P<0.05),there was statistically significant difference.Conclusion:Propofol combined with small doses of fentanyl for enteroscopy can reduce dosage of propofol,side effects such as respiratory depression and circulation inhibition, and can shorten recovery time, obtain more stable anesthesia.
