目的 探讨彩色多普勒超声产前诊断双胎输血综和征的临床价值. 方法 二维重点观察两胎儿之一有无贴附现象,两胎儿性别是否相同,两胎儿胎盘个数,两胎儿间有无丝线样的分隔,胎盘与隔膜连续处有无双胎峰,两胎儿之间有无不平衡性生长,羊水的多少,两脐带的走行及脐带有无异常,最后注意有无合并其它畸形.应用多普勒超声分别测量脐动脉收缩/舒张血流速度的比值(S/D比值)、阻力指数(RI)、波动指数(PI),并观察供血儿脐动脉舒张末期血流. 结果产前超声共筛查诊断双胎输血综和征41例,合并其它畸形6例. 结论 彩色多普勒超声能对双胎输血综和征做出产前诊断,对进一步诊断及治疗方案的选择有重要的临床价值.

Objective The purpose of this study was to evaluate the clinic value of color Doppler ultrasound in prenatal diagnosis of Twin-twin Transfusion Syndrome (TTTS). Methods Emphasis observe the stuck twin phenomena in one of the twins; the sex of the twins are same or no; the number of placenta of the two twins; are there divide like silk thread between the two twins; are there dual embryo peak on the continual place of the two placenta membranous; are there discordant twin pregnancies; the fluid volume; the anatomic variation or abnormal of the two umbilical cord; and observe the fetal combing any other malformationorno finally by two dimensional ultrasound. Assess the ratio of peak systolic to least diastolic flow velocity(S/D)、resistance index(RI) 、and pulsate index(PI) by Doppler ultrasound, and observe the send-diastolic velocity of the umbilical arteries of the donors. Results there are 41case was TTTS in the screening by prenatal ultrasound, and 6cases combing other malformation.Conclusion TTTS can be prenatal diagnosed by color Doppler ultrasound, which is of an important clinical value in the management of pregnancy
