目的 观察重症脓毒症患者血小板膜糖蛋白CD62p、CD63表达水平的变化,探讨两者与重症脓毒症严重程度及预后的关系.方法 用荧光素标志的单克隆抗体和流式细胞仪检测40例重症脓毒症患者发病后不同时间血小板糖蛋白CD62p、CD63的分子表达,采用酶联免疫吸附法(ELISA)测定发病后24 h内血清前降钙素(PCT)和C反应蛋白(CRP)浓度, 并且进行相关性分析.结果 重症脓毒症患者发病后各时间点CD62p、CD63水平较正常对照均显著升高( P均<0.01);死亡组患者各时间点CD62p、CD63水平较存活组均升高(P 均<0.05).CD62p、CD63水平与PCT、CRP呈正相关.结论 重症脓毒症患者血小板处于高激活状态, CD62p、CD63水平均显著 升高,可以作为判断疾病严重程度及预后的指标.

Objective To investigate the changes of the platelet glycoprotein CD62p、CD63 in severe sepsis patients,and their relationship with severity and prognosis of patients. Methods CD62p、CD63 expressions were measured by flow cytometry(FCM)and platelet monoclonal antibodies at dif fer ent periods after the onset of severe sepsis in 40 patients,Serum concentrations of procalcitonin(PCT) and C-Reactive Protein (CRP) were determined within 24 hours of the onset of severe sepsis. The relationship between all the de ter mi na tions and the clinical picture and prognosis of the patients was analyzed. Result The expressions of the platelet gly co pro tein CD62p、CD63 levels in severe sepsis group was significantly higher than normal group(P<0.01). CD62p、CD63 levels were higher in non-survivors (P<0.05). There was significant positive correlation between the platelet gly co pro tein CD62p、CD63 levels and PCT and CRP. Con clu sion Platelets were significantly activated in septic patients,and the degree of platelet activation was correlated with the severity of sepsis and used a prognostic marker.
