目的 观察白内障超声乳化术对单纯老年性白内障患者与合并糖尿病患者泪膜的影响.方法 将超声乳化白内障术患者80例(80只眼)分成2组:A组为单纯患老年性白内障组42例(42只眼);B组为合并患糖尿病组38例(38只眼).分别于术前和术后3,7,30,60 d观察患者干眼症状、泪膜破裂时间(BUT)、角膜荧光素染色及下睑中央泪河高度.结果 与术前相比,干眼症状在术后3,7d增加,泪膜破裂时间在术后3,7,30 d明显缩短,角膜荧光素染色在术后3,7d明显增高(P均＜0.05).2组术后干眼发病率比较有显著性差异(P＜0.05).结论 超声乳化白内障吸出术可影响泪膜的稳定性,尤其是合并糖尿病患者泪膜功能较差,术后恢复也缓慢.

목적 관찰백내장초성유화술대단순노년성백내장환자여합병당뇨병환자루막적영향.방법 장초성유화백내장술환자80례(80지안)분성2조:A조위단순환노년성백내장조42례(42지안);B조위합병환당뇨병조38례(38지안).분별우술전화술후3,7,30,60 d관찰환자간안증상、루막파렬시간(BUT)、각막형광소염색급하검중앙루하고도.결과 여술전상비,간안증상재술후3,7d증가,루막파렬시간재술후3,7,30 d명현축단,각막형광소염색재술후3,7d명현증고(P균＜0.05).2조술후간안발병솔비교유현저성차이(P＜0.05).결론 초성유화백내장흡출술가영향루막적은정성,우기시합병당뇨병환자루막공능교차,술후회복야완만.
Objective It is to approach the influence of tear film after ultrasonic phacoemulsification between simple age-related cataract and diabetic cataract. Methods 80 cases (80 eyes) underwent ultrasonic phacoemulsification were divided into 2 groups. Group A comprised 42 cases (42 eyes) with simple age-related cataract, group B comprised 38 cases (38 eyes) with diabetic cataract in. The subjective complains of dry eye, tear break-up time, corneal fluorescein stain and the height of tear meniscus were measured preoperatively and the 3rd, 7th, 30th and 60th day postoperatively. Results At the 3rd and 7th day postoperatively, the subjective complains of dry and eye corneal fluorescein stain were increased significantly (P＜0.05). At the 3rd, 7th and 30th day postoperatively, tear break-up time was reduced greatly (P＜0.05). Dry eye risks were higher in group B than that in group A. Conclusion Ultrasonic phacoemulsification significantly alters tear film of all patients in a short term postoperatively. The tear film stability of diabetic cataract is reduced in early stage after phacoemulsification and it recovers more slowly than that of age related cataract.
