目的探讨右旋美托咪啶(Dexmedetomidine,Dex)对男性病人麻醉前导尿的影响.方法择期手术男性病人60例(ASA Ⅰ~Ⅱ级),采用随机数字表随机分成Dex组和对照组:Dex组静脉泵注Dex负荷剂量1μg/kg(20 min泵完);对照组入室后以同样方式泵注等量生理盐水,泵药完成后2 min导尿.记录病人在导尿时、导尿后5 min及导尿后10 min对导尿引起的疼痛不适的视觉模拟评分(VAS评分);并比较首次导尿成功率.结果两组患者术前一般情况无显著差异.Dex组在导尿时、导尿后5 min、导尿后10 min的VAS评分明显低于对照组(P<0.05).Dex组和对照组的首次导尿成功率分别为96.7%和76.7%(P<0.05).结论右旋美托咪啶可以减轻男性病人导尿时和导尿后的疼痛不适感,并增加麻醉前导尿的首次成功率.

Objective To study the effect of dexmedetomidine(Dex) on urethral catheterization for male patients before anesthesia. Methods 60 male patients were randomized divided into dexmedetomidine group and control group. Dexmedetomidine was administered at a infusion dose of 1μg/kg in 20 minutes in Dex group, the same volume of physiological saline as placebo was administered in control group. Urinary catheter was inserted at 2 minutes after medication. Visual analogue scale(VAS) was recorded at 0,5 and 10 minutes after urethral catheterization had been done. The first-time achievment ratio of urethral catheterization between the two groups was compared.Results Before medication, the general conditions have no difference in the two groups. The VAS in the Dex group was lower than that in the control group at every time point(P<0.05).The first-time achievment ratio of urethral catheterization was 96.7% in the Dex group, which has significant difference compared with that in the control group (76.7%)(P<0.05). Conclusions Dexmedetomidine can reduce the pain and discomfort produced by urethral catheterization for male patients , and increase the achievment ratio of urethral catheterization before anesthesia.
