目的探讨医科达Synergy型医用直线加速器全碳纤维治疗床对放疗剂量的影响.方法将标准固体水模置于治疗床上,使固体水模中轴与治疗床中心纵轴重合.改变加速器机架角度,从不同角度照射并用剂量仪进行比对测量,计算出全碳纤维治疗床及其延长板对放疗剂量的衰减因子.结果6MV能量时,全碳纤维主床体对剂量的衰减在1.9%~7.2%平均衰减5.0%;延长板对剂量的衰减在0.5%~4.4%平均衰减2.2%;延长板衔接处对剂量的衰减在3.9%~43.1%平均衰减9.0%.10MV能量时,全碳纤维主床体对剂量的衰减在1.4%~6.8%平均衰减4.1%;延长板对剂量的衰减在0.3%~3.6%平均衰减1.5%;延长板衔接处对剂量的衰减在3.0%~40.1%平均衰减7.6%.结论全碳纤维床板在体部治疗的后野时床板对剂量衰减有影响,应做出修正与补偿;因延长板剂量衰减小,头部治疗时应选用延长板治疗,并考虑射野不能包含延长板衔接处,特别是110°-130°或230°-250°机架时,因衔接处有金属卡扣,剂量衰减达40.15~43.1%.相同床板6MV X射线剂量衰减率大于10MV X射线,相同能量下,剂量衰减率随床板厚度增加而增加.

Objective To investigate the effect of radiotherapy dosage caused by the ELEKTA synergy medical linear accelerator all-carbon fiber treatment couch.Methods The solid water phantom was fixed at the axis center of couch. changed the angle of the machine gantry and compare with the dosimeter measurements to calculate the all-carbon fiber treatment couch and its extension plate to the radiation dose attenuation factor. Results At 6MV X-ray, the radiotherapy dose was declined by 1.9%~7.2%,0.5%~4.4% and 3.9%~43.1% average 5.0%,2.2% and 9.0%, when the main body of the all-carbon fiber treatment couch, the head extending couch plank and department of convergence between the them were used respectively; At 10MV X-ray, the radiotherapy dose was declined by 1.4%~6.8%,0.3%~3.6% and 3.0%~40.1%average 4.1%,1.5% and 7.6%,when the main body of the all-carbon fiber treatment couch, the head extending couch plank and department of convergence between the them were used respectively; Conclusions The back-flank field impact on the dose attenuation when the all-carbon fiber treatment couch is used,and shoule be rectified and revised by dose compensate. the head extending couch plank muse be chose in the head tumor radiotherapy.in the back-lank field about 110°-130 °or 230°-250°can not contain department of convergence in extending couch with metal buckle.it woule reduce the dose by 40.15~43.1%.The penetration factor of 6MV X-ray is smaller than that of 10 MV X-ray at the same angle. The penetration factors are reducing when the thickness of the couch plank increasing,under the same enger of irradiating X-ray.
