目的探讨无创机械通气治疗急性呼吸窘迫综合征(ARDS)的临床疗效及护理经验.方法对我院2009年3月~2011年12月收治的62例早期ARDS患者,应用无创机械通气治疗,通气阶段配合心理护理,管道护理,气道湿化,预防感染等护理措施.观察患者呼吸、心率(HR)、血压(BP)等生命体征变化,并于治疗前后行动脉血气分析,观察患者血PaO2、PaCO2、SaO2、OI变化.结果经无创通气治疗后,患者呼吸、HR、BP趋于平稳,PaO2、PaCO2、SaO2、OI明显改善,与治疗前比较均具有显著性差异(P<0.01).结论早期ARDS患者及时有效的应用无创机械通气治疗及监护,能够迅速改善氧合状况,避免有创通气相关并发症,是早期ARDS重要治疗措施之一.

목적탐토무창궤계통기치료급성호흡군박종합정(ARDS)적림상료효급호리경험.방법대아원2009년3월~2011년12월수치적62례조기ARDS환자,응용무창궤계통기치료,통기계단배합심리호리,관도호리,기도습화,예방감염등호리조시.관찰환자호흡、심솔(HR)、혈압(BP)등생명체정변화,병우치료전후행동맥혈기분석,관찰환자혈PaO2、PaCO2、SaO2、OI변화.결과경무창통기치료후,환자호흡、HR、BP추우평은,PaO2、PaCO2、SaO2、OI명현개선,여치료전비교균구유현저성차이(P<0.01).결론조기ARDS환자급시유효적응용무창궤계통기치료급감호,능구신속개선양합상황,피면유창통기상관병발증,시조기ARDS중요치료조시지일.
Objective: To discuss the efficacy of non-invasive mechanical ventilation (NMV) in the treatment of acute respiratory distress syndrome (ARDS) and nursing experience. Methods: Sixty-two patients with early ARDS admitted to our hospital from March 2009 to December 2011 were treated with NMV. Meanwhile, psychological care, respiratory tract care and nursing measures for airway humidification and infection prevention were provided during ventilation period. Changes of vital signs, including respiration, heart rate, and blood pressure, were observed. And arterial blood gas analysis was performed before and after the treatment so as to observe the PaO2, PaCO2, SaO2 and OI changes in patients. Results:After NMV, patients have stabilized respiration, heart rate and blood pressure. And the situation of PaO2, PaCO2, SaO2 and OI improved remarkably. There is significant difference when compared with the situation before the treatment (P<0.01) Conclusion: Patients with early ARDS shall adopt NMV timely and efficiently. So that oxygenation status can be improved rapidly, and complications related with invasive ventilation can be avoided. Thus, NMV is an important treatment for early ARDS.
