目的探讨脑出血后抑郁(PCHD)患者甲状腺激素检测的临床意义.方法采用汉密尔顿抑郁量表(HAMD)评分将126例脑出血患者分为抑郁组(67例)和非抑郁组(59例),同时选择61例健康者作为对照组,测定对照组及脑出血后第2天、第7天、第14天、第30天游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、促甲状腺激素(TSH)水平并进行比较.结果脑出血抑郁组和非抑郁组患者,在发病第2、7、30天FT3水平均低于对照组(P<0.05),FT4和TSH水平均高于对照组(P<0.05);抑郁组病人FT3水平明显低于同期非抑郁组水平(P<0.05),抑郁组病人FT4和TSH水平明显高于同期非抑郁组水平(P<0.05);血清FT3,FT4和TSH与HAMD评分密切相关.结论脑出血患者抑郁症状可能与甲状腺功能变化有关,血清FT3、FT4和TSH可作为观察脑出血患者抑郁症状的客观指标.

Objective To observe the clinical significance of thyroid function in patients with post-cerebral hemorrhage depression(PCHD). Methods 126 patents suffering from cerebral hemorrhage were divided into two groups:Depression group(n=67) and no depression group (n=59).Thyroid function(FT3,FT4,TSH) was tested on 2nd,7th,14th and30th day respectively after stroke and compared with contrast group. Result FT3 was significantly decreased but FT4 and TSH was obviously elevated in the two groups as compared with the contrast group and the changes were more obvious in depression group than in t no depression group. The HAMD score is closely related to FT3, FT4 and TSH. Conclusion Depression symptoms occurred in the patients with cerebral hemorrhage are related with the changes of thyroid function. The levels of FT3,FT4 and TSH may be the markers of depression in patients with cerebral hemorrhage.
