目的探讨经尺骨鹰嘴关节外截骨治疗肱骨髁间C型骨折的手术方法及疗效。方法采用经尺骨鹰嘴关节外截骨入路“Y”形解剖钢板固定治疗肱骨髁间C型骨折18例。结果采用Cassebaum的方法对肘关节功能进行评价,结果本组优10例(C1型3例,C2型5例,C3型2例),良5例(C2型4例,C3型1例),可3例(C3型)。结论经尺骨鹰嘴关节外截骨是治疗肱骨髁间C型骨折较好的入路,具有易操作,有利于截骨部骨折的愈合,以及术后肘关节功能恢复并发症低等优点。

Objective To explore the surgical management and its results of type C intercondylar fracture of humerus though approach of extra-articular oblique osteomy of olecranon.Methods 18 patients of intercondylar fracture of humerus were treated surgically through approach of extra-articular oblique osteotomy of olecranon, and the fracture was fixed with plate. Results 18 patients were followed from 3 to 18 months 8 (average, 13 months), the function of elbow was assessed according to Cassebaum scale.The function of the elbow showed excellent in 10, good in 5, fair in 3. Conclusion The use of the approach of extra-articular osteotomy of olecranon for surgical management of type C intercondylar fracture of humerus has many advantages, it provides satisfactory stability with simple technical procedure avoiding intra-articular invasion, it facilitates rehabilitation exercises and provides good results with low complication rate.
