目的：研究布托啡诺对断指再植患者臂丛麻醉、术后镇痛及成活率的影响。方法上肢断指再植手术急诊患者100例，随机均分为两组，布托啡诺组（B组）和对照组（D组）。局麻药配法：2%利多卡因10 ml+0.894%甲磺酸罗哌卡因10 ml, B组加入0.1%布托啡诺1 ml, D组加入生理盐水1 ml；术后镇痛泵配法：B组布托啡诺5 mg+舒芬太尼100 ug 稀释至100 ml加入泵中，D组舒芬太尼100 ug 稀释至100 ml加入泵中。分别记录两组麻醉起效时间、痛觉消失时间、镇痛持续时间，镇痛泵持续使用时间，最后统计俩组手术的成活率。结果 B组与D组比较，痛觉消失时间更短、镇痛持续时间更长（P<0.05），镇痛泵持续使用时间更长（P<0.05），断肢再植成活率更高（P<0.05）。结论布托啡诺能够增强臂丛神经阻滞和术后镇痛泵的麻醉镇痛效果和持续时间，提高了断指再植成活率。
목적：연구포탁배낙대단지재식환자비총마취、술후진통급성활솔적영향。방법상지단지재식수술급진환자100례，수궤균분위량조，포탁배낙조（B조）화대조조（D조）。국마약배법：2%리다잡인10 ml+0.894%갑광산라고잡인10 ml, B조가입0.1%포탁배낙1 ml, D조가입생리염수1 ml；술후진통빙배법：B조포탁배낙5 mg+서분태니100 ug 희석지100 ml가입빙중，D조서분태니100 ug 희석지100 ml가입빙중。분별기록량조마취기효시간、통각소실시간、진통지속시간，진통빙지속사용시간，최후통계량조수술적성활솔。결과 B조여D조비교，통각소실시간경단、진통지속시간경장（P<0.05），진통빙지속사용시간경장（P<0.05），단지재식성활솔경고（P<0.05）。결론포탁배낙능구증강비총신경조체화술후진통빙적마취진통효과화지속시간，제고료단지재식성활솔。
Objective To observe the efficacy of butorphanol on Survival and.postoperative pain in replantation patients on brachial plexus block.Methods ASA Ⅰor Ⅱ grade were randomly divided into group B and D. Local anesthetic distribution method: They received brachial plexus blockade with 0.2% Lidocaine in 10 ml and 0.894%Ropivacaine Mesylate in 10 ml . 0.1% butorphanol in 1 ml in group B . Physiological saline In 1 ml in group D; Postoperative analgesia pump distribution method: butorphanol in 5 ml and sufentanil 100 ug diluted to 100 ml in pump in group B, Sufentanil 100 UG diluted to 100 ml in pump in group D. The time of anesthesia onset, the time to completely anesthesia, duration of anesthesia and postoperative ,the time to Continued use of analgesia pump were recorded . The survival rate of two groups of operation of the final statistics. Results B group compared with D group, the pain disappeared in shorter time, duration of analgesia (P<0.05), continuous use of analgesia pump for longer periods of time (P<0.05), the survival rate of replantation of severed limbs higher (P<0.05). Conclusion Butorphanol can enhance the analgesic effects of analgesia pump after brachial plexus block and surgery and duration, to improve the survival rate of replantation of severed finger.
