目的：评价糖皮质激素在先天性肌性斜颈（CMT）中应用的有效性和安全性。方法选择该院2011年1月至2013年1月门诊收治的100例CMT患儿（失访3例，均为对照组，因自行不来复诊被排除），采用严格的纳入标准和单盲随机分配原则，将其分为对照组（47例）和观察组（50例）：对照组采用手法治疗，包括按摩、旋转和牵伸的混合治疗的方法；观察组采用糖皮质激素配合手法治疗的方法。随访8个月至1年。结果对照组共收治47例患儿，痊愈15例，临床治愈19例，治愈率为72.3%；无效13例，转手术治疗。观察组共收治50例患儿，痊愈36例，临床治愈11例，治愈率为94.0%；无效3例，转手术治疗。观察组治愈率明显高于对照组，差异有统计学意义（P<0.05）。结论早期使用糖皮质激素配合手法治疗能够明显提高CMT患儿的治愈率，家长依从性高，临床上值得推广。

Objective To evaluate effectiveness and safety of the glucocorticoid hormone in the applications of early congenital muscular torticollis(CMT). Methods Totally 100 pediatric patients with CMT(including 3 cases were lost to follow up because of no further consultation voluntarily),who received treatment in Children′s Hospital from January 2011 to January 2013,were slected and divided into control group(n=47) and observation group(n=50) according to single-blind random distri-bution and strict inclusion criterion. The control group adopted manipulative therapy,including combined therapy of massage,ro-tation and drawing;while the observation group used glucocorticoid hormone combined with manipulative therapy. All the patients were followed up for 8 months-1 year. Results A total of 47 cases were admitted in the control group,15 cases got recovery and 19 cases cured clinical with the recovery rate of 72.3%,and the other 13 cases turned to surgical treatment because of invalidity. Totally 50 cases were admitted in the observation group ,36 cases got recovery and 11 cases cured clinically with the recovery rate of 94.0%,and the other 3 cases transferred to surgery treatment because of invalidity. The recovery rate in observation group was significantly higher than that in the control group(P<0.05). Conclusion Early application of the glucocorticoid hormone combined with the manipulative therapy can obviously increase the recovery rate of CMT with high compliance of parents ,it is worthy of clinical promotion.
