目的:分析晚断脐与常规断脐对于新生儿黄疸的影响。方法将在我院临产的360例新生儿随机分为推迟断脐组(推迟组)和常规断脐组(常规组),各180例,推迟组在120s以后进行断脐,常规组10s内断脐;将两组产后胎盘余血量、脐带出血量,Apgar 评分;产后以及产后1周新生儿胆红素水平(经皮测)、血液Hb与水平进行比较,并随访产后半个月以内新生儿黄疸发生率。结果推迟组胎盘余血量、脐带出血量明显少于常规组,产后1周外周血Hb水平高于常规组;两组Apgar评分、新生儿胆红素水平和脐血Hb水平以及随访黄疸发生率无明显差异(P>0.05)。结论晚断脐不直接诱发新生儿高胆红素血症和黄疸,并且对预防新生儿贫血有积极的临床意义。

목적:분석만단제여상규단제대우신생인황달적영향。방법장재아원임산적360례신생인수궤분위추지단제조(추지조)화상규단제조(상규조),각180례,추지조재120s이후진행단제,상규조10s내단제;장량조산후태반여혈량、제대출혈량,Apgar 평분;산후이급산후1주신생인담홍소수평(경피측)、혈액Hb여수평진행비교,병수방산후반개월이내신생인황달발생솔。결과추지조태반여혈량、제대출혈량명현소우상규조,산후1주외주혈Hb수평고우상규조;량조Apgar평분、신생인담홍소수평화제혈Hb수평이급수방황달발생솔무명현차이(P>0.05)。결론만단제불직접유발신생인고담홍소혈증화황달,병차대예방신생인빈혈유적겁적림상의의。
Objective To analyze delayed umbilical cord ligation’s effects on neonatal jaundice. Methods Randomly divide the 360 Newborns into delayed group and conventional group in our hospital,180 each,delayed group’ umbilical cord ligation got down 2 minutes after delivery while the conventional group’ umbilical cord ligation in 10 seconds after delivery. compare the two groups of postpartum placental blood, umbilical cord bleeding, Apgar score, postpartum bilirubin levels and Postpartum bilirubin levels within a week,both transcutaneous,and blood levels of Hb,and observe the incidence of neonatal jaundice after two weeks of postpartum. Results Postpartum placental blood, umbilical cord bleeding in delayed group was significantly less than the conventional group,while Hb levels of postpartum peripheral blood in 7 days is higher than the conventional group; apgar score, neonatal bilirubin levels and cord blood Hb levels and folow-ups have no significant difference in the incidence of jaundice(P>0.05).Conclusion Delayed umbilical cord ligation have no direct effects on neonatal hyperbilirubinemia and jaundice, and it has positive clinical significance in the prevention of neonatal anemia.
