目的:尽可能保存患有根分叉病变、底破坏、完全纵裂,因部位隐蔽而不易治疗的后磨牙及后磨牙冠,为遵循尽量保留天然牙的原则,减轻患者种植的经济负担及不具备种植适应症的患者,继续行使后磨牙良好的咀嚼功能,提高患牙的保存率。方法:选择患有病变的磨牙,经分根术后完善根管治疗,形成两个独立的根,最后进行联冠修复,牙体组织保存较少的患牙做桩核+联冠修复。结果:经2年观察,保存成功率96%,患牙咀嚼功能能恢复到较好状态。结论:应用分根术处理病变磨牙后再修复,能消除患牙病灶,是保存患有根分叉病变、髓底破坏的后磨牙及后磨牙残冠理想方法。
