目的：探讨单纯修补与胃部分切除术在治疗急性胃穿孔上的疗效差异。方法：收治急性胃穿孔患者84例，随机分为行胃部分切除术治疗的观察组和采取单纯修补保守治疗的对照组，观察比较两组手术时间、术中出血量、住院时间及并发症等情况。结果：观察组在术中出血量、住院时间及并发症等指标上明显优于对照组(P＜0.05)；对照组在手术时间上则要明显短于观察组(P＜0.01)。结论：单纯修补术和胃部分切除术均是治疗急性胃穿孔良好的手段，但相比而言胃部分切除术整体疗效甚佳，建议临床在选择术式时应依据患者实际情况。
목적：탐토단순수보여위부분절제술재치료급성위천공상적료효차이。방법：수치급성위천공환자84례，수궤분위행위부분절제술치료적관찰조화채취단순수보보수치료적대조조，관찰비교량조수술시간、술중출혈량、주원시간급병발증등정황。결과：관찰조재술중출혈량、주원시간급병발증등지표상명현우우대조조(P＜0.05)；대조조재수술시간상칙요명현단우관찰조(P＜0.01)。결론：단순수보술화위부분절제술균시치료급성위천공량호적수단，단상비이언위부분절제술정체료효심가，건의림상재선택술식시응의거환자실제정황。
Objective:To explore the curative effect difference of simple repair and partial gastrectomy in the treatment of acute gastric perforation.Methods:84 cases with acute gastric perforation were selected.They were randomly divided into the observation group and the control group.The observation group were treated with partial gastrectomy,and the control group were treated with simple repair.We observed and compared the operation time,intraoperative blood loss,hospitalization time and complications and other circumstances of the 2 groups.Results:In the observation group,the amount of bleeding,hospitalization time and complications were significantly better than that of the control group(P<0.05).In the control group,the operation time was obviously shorter than that of the observation group(P<0.01).Conclusion:Simple repair surgery and gastric resection are good methods in the treatment of acute gastric perforation,but in contrast,the overall effect of gastric resection is better.We suggest that doctors should select the appropriate operation mode according to the actual situation of patients.
