目的：探讨超选择双侧子宫动脉化疗栓塞治疗中晚期宫颈癌的临床疗效。方法经病理证实的98例宫颈癌患者行双侧子宫动脉灌注化疗药加碘油乳剂和明胶海绵栓塞。14例介入治疗后行手术根治术。结果98例造影显示双侧子宫动脉增粗、扭曲、病灶异常染色、血供丰富。总有效率93.9%，14例介入后2~3周手术根治术，术中出血量少，手术时间短，术中1例输尿管不全梗阻。主要不良反应为白细胞下降、恶心、呕吐、下腹坠胀痛。结论超选择子宫动脉灌注化疗加栓塞是中晚期宫颈癌理想的综合治疗措施之一，创伤小，并发症低。

Objective To evaluate the clinical therapeutic effect of super-selective bilateral uterine arterial chemoembolization therapy in the treatment of advanced cervical cancer. Methods Intra-arterial chemotherapy of bilateral uterine arteries and em-bolization of uterine arteries with iodine emulsion and gelfoam particles were performed in 98 patients with pathologically-proved cervical cancer. Radical hysterectomy was performed in 14 patients after interventional treatment. Results The angiography con-ducted during the procedure revealed markedly dilated and distorted uterine arteries, abnormal tumor staining and spiral tumor-feeding arteries in all cases. The total effective rate was up to 93.9%. 14 patients received radical hysterectomy 2~3 weeks after arterial chemoembolization. And the blood loss during the operation was much less than usual and the surgery procedure took shorter time. Ureter obstruction was found in 1 case during the operation. The main adverse effects included leukocytopenia, nau-sea, vomiting and abdominal pain. Conclusion Super-selective uterine arterial chemoembolization is one of the ideal comprehen-sive treatment measures for advanced uterine cervical cancer with smaller trauma and fewer complications.
